Obesity in the elderly has become a prominent public health issue. Due to physiological changes and multimorbidity in the elderly population, obesity presents complex clinical manifestations with markedly increased management difficulties. Based on this, National Center for Gerontology, Obesity and Diabetes Group, Chinese Diabetes Society, Chinese Medical Association organized a multidisciplinary panel of experts in obesity and formulated this consensus after multiple rounds of rigorous evaluation and iterative revisions. The consensus emphasizes that the management of geriatric obesity requires a comprehensive assessment tailored to age and overall health status, employing a patient-centered, individualized and stratified management strategy. Following the principle of "safety first, functional improvement, and steady progress", the goal is to reduce visceral fat accumulation, maintain or increase muscle mass, protect bone health, and improve physical function and health status. Special attention should be paid to sarcopenia in the elderly. Adequate intake of high-quality protein foods, combined with a comprehensive resistance exercise program tailored to individual limitations, could effectively improve muscle mass. Furthermore, weight loss medications should be selected based on the evidence of benefit for obesity-related complications or comorbidities, with close monitoring of adverse reactions.
To investigate the current application status of detection methods and reagents for islet autoantibodies in domestic medical institutions, and to assess their detection proficiency.
From October 15, 2024 to March 15, 2025, a total of 175 questionnaires were distributed to core network member units of the National Clinical Research Center for Endocrine and Metabolic Diseases, member units of the Chinese Diabetes Society and the Chinese Society of Endocrinology, member units of the China Type 1 Diabetes Alliance, and other relevant institutions. After collection, statistical analysis of detection methods for islet autoantibodies and the proportion of domestic reagents. Ten representative institutions were selected, and each was provided with 150 blinded and coded standardized evaluation samples (including 50 new-onset type 1 diabetes samples and 100 healthy donor samples). After each institution completes testing and submits the results, the national center uniformly analyzes the sensitivity, specificity and accuracy, as well as the consistency between test results from participating laboratories and those of the"gold standard"[radioligand assay (RLA) or electrochemiluminescence (ECL)]. Spearman correlation analysis was adopted to assess the correlation between the test results of participating laboratories and the gold standard measurements.
A total of 127 valid questionnaires were collected from all 31 provinces, municipalities, and autonomous regions in China. The proportion of institutions adopting chemiluminescence immunoassay(CLIA) was 76.6%(95/124), and that of domestic reagents used was 91.7%(121/132). Results from 10 institutions showed the following results: the median sensitivity values for glutamic acid decarboxylase autoantibody (GADA), protein tyrosine phosphatase-2 autoantibodies (IA-2A), insulin autoantibody (IAA), zinc transporter 8 autoantibody (ZnT8A) were 76.0%(range:38.0%-90.0%), 36.0%(range:8.0%-66.0%),24.0%(range:2.0%-50.0%), and 46.0%(range:0-54.0%), respectively; the median specificity values were 100.0%(range:98.0%-100.0%),100.0%(range:99.0%-100.0%), 98.0%(range:95.0%-100.0%),99.0%(range:98.0%-100.0%), respectively; the median accuracy values were 92.0%(range:78.0-96.7%), 78.7%(range:69.3-88.7%), 72.7%(range:66.0-83.3%), and 80.7%(range:66.0%-84.7%), respectively. The median concordance rates between test results from institutions and the "gold standard" were 95.7% (range:82.7%-100.0%) for GADA, 88.3% (range:80.7%-96.7%) for IA-2A, 87.3% (range:82.7%-92.0%) for IAA, and 94.4% (range:81.3%-96.0%) for ZnT8A. The median correlation coefficients (ρ) were 0.857 (range:0.780-0.905) for GADA, 0.829 (range:0.507-0.871) for IA-2A, 0.781 (range:0.601-0.811) for IAA, and 0.805 (range:0.448-0.887) for ZnT8A.
At the present stage in China, the mainstream detection method for islet autoantibodies is CLIA, and reagents have basically achieved localization. Overall, the detection quality for GADA is relatively satisfactory; however, the detection levels of IA-2A, InT8A, and particularyl IAA, still need further improvement.
To analyze the influencing factors associated with cirrhosis in patients with indeterminate-phase chronic hepatitis B (CHB).
CHB patients in the indeterminate phase who did not receive antiviral treatment at the Senior Department of Hepatology, the Fifth Medical Center of Chinese PLA General Hospital between January 2020 and June 2025 were retrospectively included. According to whether cirrhosis had occurred, the patients were divided into the cirrhosis group (patients diagnosed with cirrhosis) and the non-cirrhosis group (patients not diagnosed with cirrhosis). Differences in demographic characteristics, virological indicators, routine blood parameters, and liver function-related clinical indicators were compared between the two groups. Restricted cubic spline analysis was used to determine the threshold values of platelet and age in relation to cirrhosis risk. A multivariate logistic regression model was used to analyze factors associated with cirrhosis in patients with indeterminate-phase CHB.
A total of 422 patients was included. There were 182 patients in the cirrhosis group with 128 males and 54 females, aged (47±9) years; and 240 patients in the non-cirrhosis group with 146 males and 94 females, aged (43±11) years. Patients in the cirrhosis group were older than those in the non-cirrhosis group (P<0.001). The distribution of HBV DNA differed statistically between the two groups (P=0.002). Compared with the non-cirrhosis group, the cirrhosis group had a lower proportion of low HBV DNA load (<2×10³ U/ml) [14.8% (27/182) vs 29.6% (71/240), P<0.001] and a higher proportion of intermediate HBV DNA load (2×10³-<2×107 U/ml) [67.6% (123/182) vs 57.5% (138/240), P=0.032], whereas the proportion of high HBV DNA load (≥2×107 U/ml) did not differ significantly [17.6% (32/182) vs 12.9% (31/240), P=0.160]. Platelet count [M (Q1, Q3), 158 (94, 199)×109/L vs 205 (171, 244)×109/L] and HBsAg levels (log HBsAg) [2.9 (2.5, 3.2) vs 3.1 (2.7, 3.6) U/ml] were lower in the cirrhosis group than those in the non-cirrhosis group (both P<0.001). Restricted cubic spline analysis showed non-linear associations of platelet count and age with cirrhosis risk in patients with indeterminate-phase CHB (P<0.05). The corresponding threshold values were 45 years and 180×109/L of age and platelet count, respectively. Multivariate logistic regression analysis showed that decreased HBsAg levels (log HbsAg) (OR=1.99, 95%CI: 1.35-2.93), HBV DNA between 2×10³ and<2×107 U/ml (OR=2.12, 95%CI: 1.18-3.81), HBV DNA≥2×107 U/ml (OR=4.02, 95%CI: 1.80-9.00), age≥45 years (OR=2.06, 95%CI: 1.32-3.24), and platelet count<180×109/L (OR=3.99, 95%CI: 2.55-6.23) were associated with cirrhosis in patients with indeterminate-phase CHB.
Age≥45 years, platelet count<180×10⁹/L, HBV DNA≥2×10³ U/ml and decreased HBsAg are influencing factors for the progression to cirrhosis in patients with indeterminate-phase CHB who did not receive antiviral treatment.
To explore the clinical efficacy and application value of O-arm navigation-assisted zoning laminectomy in the treatment of severe thoracic ligamentum flavum ossification (TOLF).
A retrospective cohort study was conducted to analyze 83 patients with severe TOLF admitted to Zhengzhou Orthopedic Hospital from May 2019 to August 2023. There were 48 males and 35 females, the age was (50.1±6.0) years. Preoperative three-dimensional reconstruction of the ossified ligamentum flavum in the thoracic spine was performed using Mimics 21.0 software for preoperative planning. According to different treatment methods, patients were divided into the O-arm navigation-assisted zoning laminectomy group (navigation group, n=41) and the simple zoning laminectomy group (control group, n=42). The baseline data, operation time, intraoperative blood loss and total hospital stay of the two groups were compared. The modified Japanese Orthopaedic Association (mJOA) score and improvement rate were used to evaluate the changes in neurological function before the operation, 1 month, 3 months after the operation and at the last follow-up. The occurrence of complications in the patients was recorded.
The 83 patients were followed up for (20.3±6.9) months. There was no statistically significant differences in gender, age, lesion involved segments and disease duration between the two groups (all P>0.05). The operation time of the navigation group was significantly shorter than that of the control group [(134.4±29.9) vs (152.9±30.2) min, P=0.006]. There were no statistically significant differences in intraoperative blood loss [(412.2±98.6) vs (433.3±80.1) ml] and total hospital stay [(7.8±1.5) vs (7.8±1.4) d] between the navigation group and the control group (both P>0.05). The mJOA scores of the navigation group at 1 month, 3 months after the operation and the last follow-up were significantly higher than the preoperative mJOA score (all P<0.001). There was no significant difference in mJOA scores between the two groups at each time point (all P>0.05). There was no statistically significant difference in therapeutic effect between the two groups (P=0.616). There was no significant difference in the improvement rate of mJOA between two groups [(67.8%±19.9%) vs (69.8%±19.3%), P=0.760]. There were 7 cases of cerebrospinal fluid leakage after the operation in the navigation group. In the control group, there were 11 cases of cerebrospinal fluid leakage and 1 case of transient neurological dysfunction. The incidence of postoperative complications in the navigation group was lower than that in the control group [(7/41, 17.1%) vs (12/42, 28.6%), P=0.035].
O-arm navigation-assisted technology helps achieve precise decompression during surgery, shortens the operation time and enhance the surgical safety.
To analyze the clinical characteristics of children with eosinophilic gastroenteritis (EGE) and the factors associated with recurrence.
Children diagnosed with EGE at Capital Children′s Medical Center, Capital Medical University, from August 2015 to December 2023 were retrospectively enrolled, with follow-up until November 21, 2025. According to the occurrence of EGE recurrence, the children were divided into the recurrence group (children with recurrence during the follow-up period) and the non-recurrence group (children without recurrence during the follow-up period). Demographic data, clinical manifestations, laboratory findings, endoscopic and histopathological results, treatment regimens, and follow-up data were collected. The clinical characteristics were compared between the two groups, and multivariate Cox proportional hazards regression analysis was performed to identify factors associated with EGE recurrence.
A total of 44 children were included, with 16 in the recurrence group (10 males and 6 females), aged [M (Q1, Q3)] 8.1 (3.6, 13.6) years, and 28 in the non-recurrence group (16 males and 12 females), aged 5.8 (2.3, 9.7) years. The follow-up period was 65 (48, 80) months. The common clinical manifestations were abdominal pain, diarrhea, and vomiting, accounting for 70.5% (31/44), 29.5% (13/44), and 27.3% (12/44), respectively. Concomitant atopic diseases and a history of food allergy were observed in 27.3% (12/44) and 22.7% (10/44) of the patients, respectively. The recurrence group had a longer prediagnostic disease duration [21.0 (2.0, 48.0) vs 4.0 (2.0, 9.0) months, P=0.039], a higher proportion of Dupilumab use [31.2% (5/16) vs 0 (0/28), P=0.004], and higher serum total IgE levels [466 (214, 913) vs 50 (26, 165) kU/L, P<0.001] than those in the non-recurrence group. Multivariate Cox proportional hazards regression analysis showed that elevated serum total IgE levels (HR=4.22, 95%CI: 1.14-15.68) and colonic mucosal erosion or ulceration (HR=3.68, 95%CI: 1.05-12.96) were factors associated with disease recurrence in children with EGE.
Abdominal pain, diarrhea, and vomiting are common clinical manifestations of pediatric EGE, and some children have concomitant atopic diseases or a history of food allergy; Children with recurrent EGE have a longer disease duration before diagnosis and higher serum total IgE levels. Elevated serum total IgE levels and colonic mucosal erosion or ulceration are factors associated with disease recurrence in children with EGE.
To explore the clinical characteristics and long-term prognosis of nervus intermedius sectioning via a retrosigmoid approach for the treatment of nervus intermedius neuralgia (NIN). A total of 10 NIN patients treated at the First Affiliated Hospital of Fujian Medical University between May 2016 and October 2024 were included retrospectively. All patients underwent nervus intermedius sectioning through the retrosigmoid approach, with combined microvascular decompression (MVD) performed for those with concomitant trigeminal neuralgia (TN) or glossopharyngeal neuralgia (GN) based on intraoperative findings. Surgical efficacy was evaluated using the Barrow Neurological Institute (BNI) Pain Intensity Score, with a follow-up period of 14-109 months. Among the 10 patients [3 males, 7 females; mean age: (58.7±11.5) years; median disease duration: 3.0 years], pain was predominantly electric shock-like (6 cases, 60.0%) and stabbing (3 cases, 30.0%). Beyond typical deep ear pain (6 cases, 60.0%), pain also involved the periauricular area (4 cases, 40.0%), cheek (2 cases, 20.0%), root of tongue (1 cases, 10.0%) and 2/3 of the tongue (1 case, 10.0%). Intraoperative exploration identified offending vessels in 7 cases (anterior inferior cerebellar artery in 5 cases and posterior inferior cerebellar artery in 2 cases). The immediate postoperative efficacy rate (BNI Ⅰ-Ⅱ) was 90.0% (9/10). Over a mean follow-up period of (61.5±31.8) months, the long-term efficacy rate at the last follow-up was 70.0% (7/10). Complications included facial paralysis, taste impairment, and wound infection in 1 case each, with no instances of hearing loss or intracranial infection. Nervus intermedius sectioning is an effective treatment for NIN with a high immediate relief rate, although a certain risk of long-term recurrence remains. For patients with concomitant cranial neuralgias, nervus intermedius sectioning combined MVD can enhance overall therapeutic outcomes.
To evaluate the efficacy and safety of adalimumab (ADA) in pediatric patients with inflammatory bowel disease (IBD). Clinical data of IBD children who initially received ADA treatment at the Capital Center for Children′ s Health, Capital Medical University from January 2020 to August 2025 were retrospectively collected. At week 12 of ADA therapy, according to whether the children achieved clinical remission or endoscopic remission, they were divided into clinical remission group [pediatric Crohn′s disease activity index (PCDAI)<10.0 points or pediatric ulcerative colitis activity index (PUCAI)<10 points] and clinical non-remission group, endoscopic remission group [Crohn′s disease endoscopic index of severity (CDEIS)<3 points or ulcerative colitis endoscopic index of severity (UCEIS)=0 points] and endoscopic non-remission group. At weeks 12, 24 and 48, the clinical remission rate and endoscopic remission rate of the children were assessed, respectively. The baseline data of the remission group and the non-remission group at week 12 were compared, and the safety and antibody production status of ADA treatment were also evaluated. A total of 39 pediatric IBD patients were included, with 20 males and 19 females. The age at onset [M(Q1, Q3)] was 11 (6, 14) years, and the median follow-up time was 70 (19, 130) weeks. For ADA treatment at weeks 12, 24, and 48, the clinical remission rates of the children were 69.2% (27/39), 75.0% (18/24), and 73.7% (14/19), respectively; the endoscopic remission rates were 31.8% (7/22), 33.3% (1/3), and 42.9% (6/14), respectively. At week 12, the clinical remission group had a shorter baseline disease duration[12 (5, 24) vs 41 (34, 52) months, P<0.001] and lower baseline fecal calprotectin levels [560 (389, 1 800) vs 1 380 (813, 1 800) μg/g, P=0.015] compared to the clinical non-remission group; At week 4, ADA trough concentration was higher [22 (18, 32) vs 13 (9, 19) mg/L, P=0.017] than that in the non-remission group. Compared to the endoscopic non-remission group, at week 12 the endoscopic remission group had lower baseline fecal calprotectin levels [504 (369, 567) vs 1 200 (659, 1 800) μg/g, P=0.019] and fewer prior infliximab (IFX) users [28.6% (2/7) vs 80.0% (12/15), P=0.020]; while at week 4, ADA trough concentration was higher [35 (23, 35) vs 13 (10, 20) mg/L, P<0.001] than that in the endoscopic non-remission group. The incidence of adverse reactions was 13% (5/39), including alopecia, allergic reactions, and cytomegalovirus infection; only 3 cases produced relatively high antibodies (>30 ng/ml). ADA demonstrates good efficacy and safety in pediatric IBD patients.
To analyze the ultrasonic features of abdominal and retroperitoneal lipoblastoma in children. A retrospective analysis was performed on the ultrasonic images of 6 children with abdominal and retroperitoneal lipoblastoma confirmed by surgical and pathology in Tianjin Children′s Hospital from July 2019 to January 2025. There were 1 male and 5 females, with a median age of 5 years (range: 5 months to 6 years). All patients presented with a single lesion, 2 located in the retroperitoneum, 2 in the mesentery, and 2 in the greater omentum. The median maximum diameter was 98.5 mm (range: 43-130 mm), and all masses were larger than 30 mm in maximum diameter. Ultrasound findings: well-defined margins in 6 cases, lobulated morphology was seen in 4 cases. Irregular ring-shaped extensive calcification with posterior acoustic shadow was detected in 1 case, cystic areas within the masses were noted in 2 cases, small vessel penetration in the masses was visualized in 2 cases, blood flow signals within the masses were detectable in 3 cases. All 6 cases showed a predominantly hyperechoic mass with cord-like hyperechoic areas (fat-like echogenicity). Preoperative ultrasound suggested lipoblastoma in 5 cases, and teratoma in 1 case at initial diagnosis. The typical ultrasonic features of abdominal and retroperitoneal blastoma in children are solitary, large, possibly lobulated, and well-defined fat-like echo masses, with certain characteristic ultrasonic manifestations. Combined with clinical characteristics such as age, ultrasound as a safe and non-invasive imaging method, can be used as a routine examination for screening and the main method for long-term follow-up.
A rare case of composite pheochromocytoma (PCC) with ganglioneuroblastoma (GNB) was reported in this article, and the patient′s diagnostic and treatment course—including biochemical evaluation, imaging findings, surgery, pathology, genetic testing, and postoperative follow-up—was retrospectively analyzed. Relevant literature in PubMed, Embase, CNKI, and Wanfang Data from database inception to October 1 2025 was systematically searched to identify comparable adult cases, and their clinicopathological features were summarized. The patient was a 48-year-old man who presented to the Department of Endocrinology, First Medical Center of Chinese PLA General Hospital on March 25, 2025, with hypertension for 3 months and a left adrenal mass detected 12 days earlier. Biochemical testing showed markedly elevated normetanephrine (NMN). MRI and 18F-NOTATATE somatostatin receptor positron emission tomography-computed tomography (18F-NOTATATE-PET-CT) revealed a cystic-solid mass in the left adrenal gland, with increased uptake in the solid component [The maximum standardized uptake value (SUVmax) was 9.4]. Robot-assisted laparoscopic tumor resection was performed on the patient. Composite PCC, consisting of a pheochromocytoma component and a GNB component, was confirmed by postoperative pathology. No clearly pathogenic variants were detected by germline whole-exome sequencing. After surgery, blood pressure was normalized without antihypertensive medication, and no evidence of recurrence or metastasis was observed at the 6-month follow-up. The literature review identified 14 adult cases of PCC with GNB; together with the present case, 15 cases were analyzed. The age [M(Q1, Q3)] was 49 (35, 55) years. Hypertension was documented in 11 patients, elevated catecholamines or their metabolites were observed in most cases, tumors>5 cm in maximum diameter were found in 8 patients, and hereditary syndromes were identified in 3 patients. Composite PCC with GNB in adults is exceedingly rare. Its clinical, biochemical, and imaging manifestations frequently overlap with those of conventional PCC, rendering definitive diagnosis dependent on histopathological examination. Prognosis appears to be correlated with tumor components, size, and genetic syndrome status. Therefore, comprehensive postoperative pathological evaluation, germline genetic testing, and long-term surveillance are strongly recommended.
This paper reports two cases of hereditary angioedema (HAE) with abdominal pain and ascites as the first manifestation. Case 1, a 54-year-old male, was seen for recurrent distension and pain in the right lower abdomen, facial and lip edema, and imaging examination showed transient ascites and edema of the small intestinal wall. Case 2, a 58-year-old female, was seen for recurrent periumbilical colic and swelling of the face and lips. Both patients had decreased complement C4, decreased C1 esterase inhibitor (C1-INH) concentration and function, and were diagnosed as HAE-1. Case 1 was initially treated with danazol, but severe drug-induced liver injury occurred 1 month later. After discontinuation, it was switched to ranalimab, and there was no recurrence until 13 months after treatment (30 Apr 2026). Case 2 did not receive standardized preventive treatment due to economic and adverse drug reactions, and only relied on glucocorticoids and antihistamines for symptomatic treatment at each episode. After follow-up until April 30, 2026, the symptoms still occurred frequently. The above cases suggest that for patients with unexplained recurrent abdominal pain accompanied by skin edema, early detection of complement C4 and C1-INH can diagnose HAE in time, and standardized long-term prevention can help improve the prognosis of patients.
Cortisol is a steroid hormone secreted by the adrenal cortex. It is regulated by the negative feedback loop of the hypothalamus-pituitary-adrenal axis and has a circadian rhythm. Its level fluctuation is closely related to various physiological and pathological states. This paper systematically reviews the physiological regulation mechanism of cortisol and its important role in various disease states, focuses on the technical limitations faced by traditional detection methods (such as enzyme-linked immunosorbent assay, immunoassay, liquid chromatography-tandem mass spectrometry, etc.) in clinical application, and sorts out the research breakthroughs of wearable sensors, microfluidics, biosensing and microdialysis in dynamic monitoring of cortisol by focusing on the progress of continuous monitoring technology based on non-invasive or minimally invasive samples such as sweat and tissue fluid in recent years. Free cortisol levels in sweat and tissue fluid can effectively reflect the circadian rhythm of serum cortisol, which provides a new technical path for dynamic monitoring of Cushing's syndrome, adrenocortical hypofunction, obesity and mental health assessment. However, the current continuous cortisol monitoring technology still faces challenges such as unstable sample supply, individual differences, long-term sensor stability and lack of diagnostic criteria. In the future, with the integration of artificial intelligence, big data and precision medicine, continuous cortisol monitoring equipment is expected to realize the transformation from laboratory research to clinical application, and become an important tool for chronic disease management and individualized health monitoring.
Solitary fibrous tumor (SFT) is a class ofNAB2-STAT6Fusion is a characteristic and rare mesenchymal tumor of the central nervous system with significant recurrence characteristics and a tendency to distant metastasis. Because of the complex anatomical structure of skull base, adjacent to important nerve vessels, and abundant blood supply of tumor, surgical resection is difficult, so it is challenging to achieve maximum safe resection. This article systematically reviews the clinical characteristics, key points of imaging and pathological diagnosis, surgical treatment strategies and perioperative management principles of skull base SFT, focusing on the role of prognostic factors such as extent of resection, World Health Organization classification, mitotic count, Ki-67 index and recurrence history in risk stratification, and summarizes the research progress of postoperative radiotherapy, stereotactic radiosurgery and systemic therapy. Combined with the biological characteristics of late recurrence and late metastasis of skull base SFT, a long-term follow-up and comprehensive management strategy based on risk stratification was proposed to provide reference for individualized treatment of SFT.
On the evening of May 8, 2026, the mother of the patient Xiao L sent me the video of Xiao L walking and exercising independently in the rehabilitation hospital, and thanked me for my assistance. I was as moved as this mother: this 22-year-old girl who was unfortunately involved in a major car accident suffered from bifrontal brain contusion and laceration, and severe fractures of skull base and craniorbital surface. Although she underwent bifrontal decompressive craniectomy, she experienced a series of secondary central and peripheral key organ injuries such as severe ischemic cerebral edema, hypothalamic injury, secondary neurogenic pulmonary edema, liver dysfunction and severe water and electrolyte disorder less than 3 days after the operation. I was contacted by a peer to assess whether the young patient had a silver lining of survival. According to the limited information of imaging data and laboratory examinations provided by local doctors, I can only judge that patients with such critical illness should be referred to the neurosurgical critical care platform with higher diagnosis and treatment level and better equipment and facilities for treatment. However, when I suggested that she be transferred to Xuanwu Hospital for treatment, the local doctor in charge considered the patient's safety, worried about the risk of death during the transfer, and tried her best to prevent the long-distance transfer of patients. However, we believe that if local doctors believe that the patient has no hope and continues to stay in the local area for treatment, the patient's hope of recovery will be even slimmer. We explained the pros and cons to the family members in detail on the phone, and the family members finally made up their minds to take the risk of transporting the patient. After being transferred to our department, after active treatment, it took only 5 days for the patient's consciousness to basically recover; After 2 weeks of treatment such as adjusting liver function and correcting electrolyte disorder, I suggested that she should be transferred to a well-known specialist hospital for optic canal decompression to save her vision in combination with the serious damage of her vision in both eyes. However, after seeing her condition, the receiving doctor made a judgment that although the patient's consciousness had recovered, the systemic complications were serious, and it was of little significance to save her vision. Hearing the news, the patient's family nearly collapsed. But based on the patient's clinical manifestations and consciousness, we judged that she should have a good outcome, and encouraged the mother to undergo this vision-saving operation, and the results confirmed our judgment.
