Extracorporeal cardiopulmonary resuscitation (ECPR) refers to the use of extracorporeal membrane oxygenation (ECMO) to ensure the perfusion of important organs after the traditional cardiopulmonary resuscitation (CCPR) has not obtained the return of spontaneous circulation (ROSC). Such cardiopulmonary resuscitation (CPR) is called ECPR. ECPR can benefit some patients with cardiac arrest, however, there are still some problems and puzzles in the implementation of ECPR, such as the implementation location of ECPR patients? Select mechanical press or manual press before ECPR start? Can ECPR be used in special patients, such as traumatic cardiac arrest (TCA), aortic dissection and immunosuppressed patients? The age limit of ECPR and the ethical issues related to ECPR. Based on the research status at home and abroad, this paper analyzes and expounds these problems, hoping to provide new ideas for the research and application of ECPR by the majority of domestic colleagues engaged in cardiopulmonary resuscitation.
The rapid development of big data methods and technologies has provided more and more new ideas and methods for clinical diagnosis and treatment. The emergence of large language models (LLM) has made it possible for human-computer interactive dialogues and applications in complex medical scenarios. Critical care medicine is a process of continuous dynamic targeted treatment. The huge data generated in this process needs to be integrated and optimized through models for clinical application, interaction in teaching simulation, and assistance in scientific research. Using the LLM represented by generative pre-trained transformer ChatGPT can initially realize the application in the diagnosis of severe diseases, the prediction of death risk and the management of medical records. At the same time, the time and space limitations, illusions and ethical and moral issues of ChatGPT emerged as the times require. In the future, it is undeniable that it may play a huge role in the diagnosis and treatment of critical care medicine, but the current application should be combined with more clinical knowledge reserves of critical care medicine to carefully judge its conclusions.
Liver transplantation is currently the only effective treatment for end-stage liver disease. Ultrasound examination has the advantages of non-invasive, convenient, and real-time display the intrahepatic vascular hemodynamic changes, and has become one of the preferred evaluation methods in the entire management process of liver transplantation. To further standardize the ultrasound examinations before and after liver transplantation, multiple-disciplinary experts in the field of liver transplantation, including specialists in ultrasound, liver surgery, and intervention medicine, were organized by the Chinese Medical Association Ultrasound Medicine Branch, the Chinese Medical Doctor Association Surgery Physician Branch, the Chinese Medical Doctor Association Interventional Physician Branch, and the Shanghai Medical Association Ultrasound Medicine Branch. Combined with the latest research practice in ultrasound diagnosis and treatment of liver transplantation, the ultrasound clinical practice guidelines for liver transplantation (2023 edition) was formulated. The contents of this guideline include ultrasound evaluation of liver transplantation donors and recipients, intraoperative and postoperative ultrasound examination of transplant livers, ultrasound applications in pediatric liver transplantation, and ultrasound-guided liver puncture biopsy and treatment, aiming to promote the standardized and high-quality development of liver transplantation ultrasound examination in China.
The genes encoding human leukocyte antigens (HLA) are located in the region of 6p21.31 of the human chromosome, accounting for 1/3 000 of the human genome. The area is seen as a structurally complex, with the characteristics of multi-gene inheritance, high polymorphism, haplotype inheritance, linkage disequilibrium, racial diversity, and regional distribution. In recent years, with the continuous innovation of HLA genotyping technology, new issues and challenges have emerged in terms of the standardization of the technology platform, the implementation of the performance validation and verification plan for clinical testing projects, quality control of the entire testing process, analysis and interpretation of results, consultation and selection of donors, and clinical application. This consensus, in combination with the current status of HLA genotyping technology in clinical practice in China, systematically sorts out the key contents in the standardization and clinical application of the HLA genotyping technology platform. The first part proposes the basic requirements for the standardization of the HLA typing technology platform from the perspectives of personnel, instrumentation, reagents and consumables, experimental methods, environmental settings, and introduces the methods and performance indicators for confirming the performance of HLA genotyping with next-generation sequencing (NGS) technology. The second part elaborates on the key points of quality control during the entire process of HLA genotyping procedure, quality control of result reporting and releasing, and management of result interpretation and bioinformatics systems. The last part describes the clinical consultation and application promotion of HLA genotyping. After widely soliciting revisions and discussions from domestic experts in the field of HLA and transplantation, the consensus aims to promote the development and clinical application of the HLA gene in related medical field.
To investigate the correlation of spinopelvic alignment according to Roussouly classification on the occurrence of adjacent segment disease (ASD) in the patients undergoing fusion surgery for lumbar degenerative diseases.
A cross-sectional study. Clinical data of 166 consecutive patients who had undergone lumbar fusion between January 2009 and January 2019 in the Affiliated Changzhou Second People′s Hospital of Nanjing Medical University and Affiliated Drum Tower Hospital of Medical School of Nanjing University were retrospectively reviewed. There were 59 males and 107 females, with an average age of (58.6±9.5) years (ranged 41-78 years). Fusion length averagely spanned (1.7±0.7) levels (ranged 1-3 levels). The patients were classified by both "theoretical" (based on pelvic incidence (PI)) and "current"(based on sacral slope (SS)) Roussouly types. The patients were classified as "matched" if their "current" shape matched the "theoretical" type and otherwise as "unmatched". Multivariate logistic regression analysis of the variables recruited from univariate analyses was performed to identify the factors independently associated with the development of ASD after lumbar fusion.
The average follow-up duration after initial surgery was (49.2±20.7) months (ranged 25 to 134 months). Thirty (18.1%, 30/166) patients were diagnosed as ASD. Postoperatively, two thirds of the patients who suffered ASD after surgery were unmatched, while 36.8% (50/136) of the patients without ASD had unmatched type. Univariate analyses showed that older age, more fusion levels, float fusion, pre-and postoperative worse spinopelvic alignment, and postoperative unmatched Roussouly type were identified as risk factors of ASD. Multivariate logistic regression analysis identified postoperative Roussouly type mismatch (OR=3.310, 95%CI: 1.282-8.545, P=0.013), old age (OR=1.074, 95%CI: 1.019-1.131, P=0.008) and postoperative SS (OR=0.928, 95%CI: 0.865-0.995, P=0.036) as the independent risk factors of development of ASD after lumbar fusion.
A significant association between postoperative sagittal malalignment and occurrence of ASD is detected, the evaluation of sagittal alignment by Roussouly classification could help predict the occurrence of ASD.
To evaluate the effect of compound chamomile and lidocaine hydrochloride gel on oropharyngeal complications after the use of laryngeal mask airway with positive pressure ventilation.
A total of 64 patients undergoing elective surgery under general anesthesia using laryngeal mask airway (LMA) with positive pressure ventilation in Beijing Tongren Hospital Affiliated to Capital Medical University From January to March 2023 were prospectively selected and divided into two groups by the random number table method: normal saline group (n=32), with 18 males and 14 females, aged 28-64 (48.4±10.6) years, and the compound chamomile and lidocaine hydrochloride gel group (chamomile gel group, n=32), with 18 males and 14 females, aged 24-64 (46.3±10.8) years. Patients in both groups received total intravenous anesthesia. Before LMA placement, the front, shoulder and back of LMA were lubricated with normal saline and compound chamomile and lidocaine hydrochloride gel in the normal saline group and the chamomile gel group, respectively. The main outcome measure was the incidence of postoperative oropharyngeal mucosal inflammation at different time points after the removal of the laryngeal mask. Secondary outcome measures included oropharyngeal mucositis score, sore throat score, hoarse voice score, cough score and throat dryness score at different time points after LMA removal, the number of LMA insertion times, time of successful insertion and leakage pressure, as well as stress reactions such as cough and agitation before and after LMA removal, and adverse reactions such as tongue numbness and protective pharyngeal reflex limitation during recovery.
The incidence of oropharyngeal mucositis in the chamomile gel group was 25.0% (8/32), 31.3% (10/32), 28.1% (9/32) and 3.1% (1/32) at 0.5, 3, 6 and 24 h after LMA removal, respectively, which were lower than those of normal saline group [53.1% (17/32), 59.4% (19/32), 59.4% (19/32) and 21.9% (7/32)] (all P<0.05). The oropharyngeal mucositis scores of patients in the chamomile gel group were 0 (0, 0.8), 0 (0, 1.0), 0 (0, 1.0) and 0 (0, 0) respectively at 0.5, 3, 6 and 24 h after LMA removal, which were lower than those in the saline group [1.0 (1.0, 1.8), 1.0 (0, 2.0), 1.0 (0, 2.0) and 0 (0, 0)] (all P<0.05). The cough score of the patients in the chamomile gel group was 0 (0, 0) and 0 (0, 0) at 3, 6 h after LMA removal, which were lower than those of the normal saline group [(0, 0) and 0 (0, 0)] (both P<0.05). The throat dryness score of the patients in the chamomile gel group was 0 (0, 1.0) at 3 h after LMA removal, which was lower than that of the normal saline group [1.0 (0.3, 1.0)] (P=0.019). The time of successful LMA insertion in the chamomile gel group was 25.0 (20.3, 29.8) s, which was shorter than that in the saline group [29.0 (25.0, 32.0) s] (P=0.016). There were no significant differences in the number of LMA insertion, leakage pressure, postoperative sore throat and hoarse voice scores between the two groups (all P>0.05). Likewise, there were no stress reactions such as cough and agitation before and after LMA removal, and no adverse reactions such as tongue numbness and limited protective pharyngeal reflex during recovery.
Compound chamomile and hydrochloride lidocaine gel can reduce the incidence of postoperative oropharyngeal mucositis, relieve the symptoms of postoperative oropharyngeal mucositis, pharyngeal dryness and cough, and improve the postoperative comfort of patients using the laryngeal mask airway with positive pressure ventilation.
To investigate the diagnostic performance of multiparametric dynamic contrast-enhanced MRI(DCE-MRI) for the differentiation between benign and malignant larcrimal gland epithelial tumors.
The clinical and imaging data of 104 patients with epithelial tumors of the lacrimal gland who underwent orbital MRI scan and met the inclusion criteria in Beijing Tongren Hospital from January 2011 to December 2017 were retrospectively collected, including 48 males and 56 females, aged from 12 to 77 (43±7) years. Sixty-three cases of benign epithelial tumors and 41 cases of malignant epithelial tumors were examined by DCE-MRI. The parameters of semiquantitative analysis including: time to peak enhancement (Tpeak), maximum enhancement ratio (ERmax), Slope, washout ratio (WR) and time-signal intensity curve (TIC) types. The parameters of quantitative analysis including: volume transfer constant (Ktrans), the extravascular extracellular volume fraction (Ve) and rate constant (Kep). Receiver operating characteristic (ROC) curve analysis was performed for DCE-MRI parameters with statistically significant differences, the area under the curve (AUC) was calculated, the diagnostic threshold was determined, and the diagnostic performance was evaluated. Logistic regression analysis was used to determine the best parameters for differential diagnosis of benign and malignant epithelial tumors of the lacrimal gland.
For the semiquantitative analysis of DCE-MRI, malignant lacrimal gland epithelial tumor had a significantly shorter Tpeak than benign masses [(103.77±57.87) s vs (187.80±77.01) s,P<0.001)], while had a higher value in ERmax, Slope [M(Q1,Q3)] and WR in malignant masses compared with benign one [1.55±0.39 vs 1.36±0.33; 1.76 (0.97,2.27) vs 0.62 (0.50,0.93); 7.70%(1.40%, 21.60%)% vs 0(0, 0),all P<0.05)].The TICs of benign lacrimal tumors mainly showed a persistent type (49/63),while most malignant lacrimal tumors mainly showed a plateau type (25/41). For the quantitative analysis of DCE-MRI, the values of Ktrans and Kep[M(Q1,Q3)] in malignant tumors were significantly greater than those of benign tumors (0.99±0.52/min vs 0.43±0.23/min, P<0.001; 1.33(0.83, 1.55)/min vs 0.55(0.46, 0.68)/min, P<0.001). No significant difference in Ve was found between the groups (0.76±0.20 vs 0.73±0.22,P=0.467). Through the statistical analysis, TIC types (OR=3.887,95%CI: 1.409-10.725) and Ktrans(OR=50.979,95%CI: 6.046-429.830) can provide superior diagnostic performance for predicting malignant lacrimal gland epithelial tumors, with a sensitivity of 78.05%, specificity of 77.78%,and sensitivity of 70.73%, specificity of 95.24%, respevtively. Furthermore, the comprehensive diagnostic performance of Ktrans in AUC was proven to be significantly better than that of TIC [0.875 (0.796-0.932) vs 0.798 (0.708-0.870),P=0.049].
Multiparametric DCE-MRI is helpful for the differential diagnosis of benign and malignant epithelial tumors of lacrimal gland. TIC type and Ktrans have higher diagnostic value, and the diagnostic performance of Ktrans is better than that of TIC.
To investigate the value of qualitative and quantitative PET/MRI in the evaluation of axillary lymph node metastasis in patients with breast cancer.
A total of 33 patients with breast cancer underwent preoperative PET/MRI examinations in Jingling Hospital from February to August in 2022 were retrospectively collected. All these patients were female, aged from 34 to 73 (51.4±11.3) years. Histopathological results and follow-up data were deemed as the referent standard, and the images were independently evaluated by two experienced breast imaging radiologists. The qualitative PET/MRI evaluation procedures were designed to evaluated the MRI alone to classify the axillary lymph nodes firstly, and then, the axillary lymph nodes status was reclassified by combining MRI and PET images. The net reclassification improvement index (NRI) was calculated using the R Programming Language (RStudio). The quantitative PET/MRI evaluation of the maximum standard uptake value (SUVmax) of axillary lymph nodes were measured by two radiologists, respectively, and the average value was compared with the referent standard to conduct a receiver operating characteristic (ROC) curve to select the optimal cutoff value of SUVmax. Based on the cutoff value and MRI classification results, axillary lymph nodes status was divided into quantitative PET/MRI positive or negative. The sensitivity, specificity and accuracy of MRI and quantitative PET/MRI in evaluating axillary lymph node metastasis were compared, and the area under the ROC curve (AUC) was compared.
There was no significant difference in sensitivity, specificity and accuracy between MRI and quantitative PET/MRI in evaluating lymph node metastasis of breast cancer (81.82% vs 95.46%; 81.82% vs 100%; 81.82% vs 96.97%) (all P>0.05). The AUC had a statistically significant difference [0.82 (0.65 to 0.93) vs 0.98 (0.85 to 1.00), P=0.026)]. According to the referent standard, in the 11 cases without ipsilateral axillary lymph node metastasis, the SUVmax was 0.83±0.18, while in the 22 cases with ipsilateral axillary lymph node metastasis, the SUVmax was [4.36 (1.77, 5.85)]. Compared with MRI alone, the NRI of qualitative PET/MRI in evaluating lymph node metastasis was 36.36% (P=0.021).
Compared with MRI alone, quantitative PET/MRI has a higher AUC for evaluating axillary lymph node metastasis in patients with breast cancer, and qualitative PET/MRI had a better reclassification power in the evaluation of axillary lymph node metastasis.
To investigate the efficacy and safety of high-voltage pulse radiofrequency combined with pregabalin on severe thoracic postherpetic neuralgia (PHN).
A total of 103 patients with PHN who were admitted to the Department of Pain Medicine of Henan Provincial People′s Hospital from May 2020 to May 2022 were retrospectively selected, including 50 males and 53 females, and aged 40 to 79 (65.4±9.2) years. The patients were divided into two groups according to the treatment methods they received: the control group (n=51) and the study group (n=52). The patients in the control group were treated with oral pregabalin, and the patients in the study group received pregabalin plus high-voltage pulse radiofrequency therapy. The pain intensity and efficacy of the two groups were evaluated before treatment and 4 weeks after treatment. The pain intensity, the sleep quality and the efficacy of treatment was evaluated by visual analogue scale (VAS) score, Pittsburgh Sleep Quality Index (PSQI) score and nimodipine method, respectively. The levels of pain mediators including serum neuropeptide Y (NPY), prostaglandin E2 (PGE2), substance P (SP) and β-endorphin were measured. The differences of the above indicators and the incidence of adverse reactions were compared between the two groups.
The VAS scores of the study group and the control group before treatment were 7.94±0.76 and 8.20±0.81, and PSQI scores were 16.84±3.90 and 16.29±3.84, respectively, with no statistically significant differences (both P>0.05). After 4 weeks of treatment, the VAS scores of the two groups were 2.84±0.80 and 3.35±0.87, and PSQI scores were 6.78±1.90 and 7.98±2.40, respectively, and the VAS score and PSQI score in the study group were lower than those in the control group (both P<0.05). There were no significant differences of the serum levels of NPY, PGE2, SP and β-endorphin before treatment in the study group and control group (all P>0.05). After 4 weeks of treatment, the levels of NPY, PGE2, SP and β-Endorphin in the study group were (240.7±26.8) ng/L, (74.4±8.6) μg/L, (108.9±15.7) ng/L and (4.4±0.9) ng/L, which were lower than those in the control group [(268.1±29.4) ng/L, (79.7±8.3) μg/L, (115.2±16.2) ng/L, (5.2±1.3) ng/L, respectively], with statistically significant differences (all P<0.05). After treatment, 29 cases were cured, 16 cases were markedly effective and 6 cases were effective in the study group, while 16 cases, 24 cases and 8 cases were cured, markedly effective and effective in the control group, respectively. The overall efficacy of patients in the study group was better than that in the control group (Z=-2.32, P=0.018). The incidence of adverse reactions in the study group and control group was 11.5% (6/52) and 7.8% (4/51), respectively, with no statistically significant difference (χ2=0.40, P=0.527).
High-voltage pulse radiofrequency combined with pregabalin can significantly improve the pain intensity and sleep quality of patients with severe thoracic PHN and reduce the levels of pain mediators, with a high safety profile.
Sixteen patients with recurrent high-grade glioma who were treated by domestic magnetic resonance-guided laser interstitial thermotherapy (MRgLITT) in the Neurosurgery Department of Xuanwu Hospital of Capital Medical University from 1st January 2021 to 31st December 2021 were prospectively included, with 11 males and 5 females, and aged 27-74 (50±16) years. The duration of surgery, the rate of ablation after surgery, and perioperative complications were assessed. The patients were followed up every 3 months to assess survival and progression. A total of 5 WHO grade Ⅲ patients and 11 WHO grade Ⅳ patients were included. The operation time was 144 (109, 176) min, 28 targeted lesions were detected, and the ablation rate [M (Q1, Q3)] was 91.0% (87.4%, 93.3%). After surgery, 2 patients (2/16) had decreased limb muscle strength, and no perioperative death or other serious complications occurred. The median time to a complete response was 12 (5, 14) months in WHO Grade Ⅲ patients, and one died 12 months after surgery, while the median time to a complete response was 3 (1, 8) months in 11 WHO Grade Ⅳ patients, with a total of 8 deaths at the last follow-up. Therefore, domestic MRgLITT has certain efficacy and safety in the treatment of recurrent high-grade glioma, providing a new option for patients with recurrent glioma.

The clinical success rate of the internal stents was 12/12, with the median retention time of [M(Q1, Q3), 64.5 (52.0, 90.8)] days. The postoperative displacement rate and severe stenosis incidence were 1/12 and 3/12, respectively. The esophageal stents were successfully removed in one endoscopic session in all patients. A small amount of mucous membrane extravasation occurred in all patients after SIS, and no patients died after 90 days of follow-up.
Tandem lesions are arteries with severe stenosis or occlusion proximal to their occlusive arteries in patients with acute large vessel occlusive stroke. Tandem lesions have a higher incidence and a worse prognosis than isolated arterial occlusion. A case of tandem lesion with successful vascular opening after endovascular therapy 3 d after onset is reported. The patient was a 56-year-old male who visited the emergency department mainly for "transient slurred speech for 4 d and recurrent for 3 d, right limb weakness for 1 d, and aggravation for 13 h." The National Institutes of Health Stroke Scale (NIHSS) score was 13. Whole brain angiography showed occlusion of left internal carotid artery and middle cerebral artery, and successful vascular opening after endovascular treatment. At the 3-month postoperative follow-up, the patient's modified Rankin scale (mRS score) was 3.
In December 2022, the health industry standard WS/T 404.10-2022 "Reference Interval of Commonly Used Clinical Biochemical Testing Items Part 10: Serum Triiodothyronine, Thyroxine, Free Triiodothyronine, Free Thyroxine and Thyroid Stimulating Hormone" was released. The formulation of this standard is based on relevant domestic and foreign standards and guidelines, large-sample multi-center research data of healthy adults in China, national verification data as reference, and widely solicited the opinions of clinical, laboratory, epidemiological and statistical experts. The aim is to provide medical institutions at all levels in China with reference intervals for thyroid-related hormones suitable for adults in China, and provide scientific basis for the health assessment of thyroid function and screening, diagnosis and treatment monitoring of thyroid diseases in adults. This article interprets the key clauses such as the verification and use of reference intervals in clinical laboratories, the precautions for changing reference intervals, and the inconsistent clinical interpretation of test results before and after the change of reference intervals, so as to help clinicians and relevant laboratory staff better understand and apply this standard.
During cardiopulmonary resuscitation (CPR), it is difficult to judge the restoration of spontaneous circulation (ROSC) by touching the arterial pulsation. It is necessary to interrupt chest compression during examination, and the judgment results are often unreliable. With the development of physiological parameter monitoring technology and the continuous development of prediction models for the possibility of ROSC in patients with sudden arrest, many faster and more accurate methods have been applied to identify and predict ROSC during CPR. In this paper, the methods used to identify and predict ROSC in CPR are reviewed for reference in related fields.
Empirical evidence suggests a significant two-way association between mental disorders and somatic disorders, but there are no studies outlining in detail the potential impact of mental disorders on clinical outcomes of somatic disorders. In this umbrella review consistent with PRISMA and COSMOS-E, the authors retrieved Systematic Reviews and meta-analysis literature on the prospective association between any psychiatric disorder and clinical outcome of somatic disease in databases such as PubMed, PsycINFO, Embase, and Joanna Briggs Institute Database of Systematic Reviews and Implementation Reports up to March 15, 2022. Primary outcomes included disease-specific mortality and all-cause mortality. Secondary outcomes include disease-specific incidence, function and/or disability, symptom severity, quality of life, relapse or progression, major cardiac events, and treatment-related outcomes. A random effects model was adopted, combined with I2Statistics, 95% prediction interval, small sample effect test, excessive significance bias test, and risk of bias (ROBIS) assessment. According to the established criteria, the associations were classified into five levels of credibility evidence (Ⅰ ~ Ⅳ and non-significant), and the robustness of the primary analysis was tested by sensitivity and subgroup analysis. Statistical analysis was performed using the Umbrella Review new software package (https: / /metaumbrella.org). Then, the population attributable score (PAF) and generalized impact score (GIF) for category Ⅰ ~ Ⅲ associations were calculated.
