Artificial joint arthroplasty, such as total hip and total knee arthroplasty, is one of the most successful procedures in orthopedics. The quality of life of these patients can be improved significantly through relieving pain, correcting the deformity and improving joint function. Although hip and knee arthroplasty currently are routine surgeries for patients with late stage joint diseases, post-operative complications like leg length discrepancy, dislocation or instability, limp and uncertain reason pain are still common. The position of the implants plays important roles in all these complications. Robotic assisted joint arthroplasty can not only provide 3D pre-operative visual planning, but also can assist to fulfill the pre-operation plan perfectly. But the current joint arthroplasty robots cannot carry out automatic personalized pre-operation planning, and also have the shortcomings of low efficiency and high cost etc. However, based on their current performance and with the help of artificial intelligence and better algorithm, we believe that joint arthroplasty will be more and more precisive, intelligent and minimally invasive, which is an inevitable trend in the development of joint surgery.
Digital orthopedics technology is a new interdisciplinary subject, known as the third technological wave in the field of orthopedics. The development direction of arthroplasty is focused on hoping to improve the long-term prognosis of patients by imitating more natural kinematics and optimizing implant positioning strategies. At present, various digital orthopedic technologies such as virtual reality technology, navigation assistance systems, and patient-specific instrumentation (PSI) and robot-assisted surgery in joint surgery are in full swing. In this article, relevant domestic and foreign literature were extensively reviewed and the author′s own experiences were summarize, and the advantages, disadvantages, clinical efficacy and future prospects of the above-mentioned various technologies were evaluated too.
Stroke poses a social and economic burden worldwide.Intravenous thrombolytic therapy and endovascular interventional therapy are recommended as early as possible for patients with acute ischemic stroke in many national and international guidelines, however, their clinical applications are limited due to their strong time dependence. To date, the treatment of acute stroke in China has many problems, such as backward development of hospital treatment process and shortage of stroke professionals.Establishing a complete stroke green channel and maintaining its smooth operation contributes to the most important and effective way to promote thrombolytic therapy, which requires setting a clear target time, appropriately adjusting the hospital layout and hardware and software investment, attaching importance to team building and clear job responsibilities. Moreover, the most important task is to improve the green channel process through replacing the "serial mode" with the "parallel mode", making full use of the first aid map of stroke, bridging the gap between pre-hospital and in-hospital treatment, and popularizing stroke-related knowledge. In recent years, considerable progress has been made in the construction of stroke green channel in China. The implementation of the above-mentioned reform mode may minimize pre-hospital and in-hospital delays, expand the benefit population of stroke and thus improve the early treatment rate of acute ischemic stroke.
Eosinophilia-associated lung disease (EPD) refers to a heterogeneous group of clinical diseases characterized by increased eosinophils (EOS) in the airways and/or lungs (parenchyma, interstitium, alveoli) and/or pleural cavity, with or without increased peripheral blood EOS. The clinical evaluation and diagnostic decision-making of EPD are challenging issues, and there is no consensus of EPD diagnosis and treatment experts in China. The National Respiratory Medicine Center of the First Affiliated Hospital of Guangzhou Medical University, the National Clinical Medical Research Center of Respiratory Diseases and the Asthma Group of Respiratory Diseases Branch of Chinese Medical Association extensively solicited the opinions of multi-disciplinary experts at home and abroad, drawn on and referred to the existing international EPD diagnosis and treatment guidelines and clinical research, and combined with the experience of EPD diagnosis and treatment in China, this consensus was formulated, and the pathogenesis, diagnostic criteria, classification, etiology, evaluation and treatment of EPD were systematically introduced, aiming at helping clinicians to formulate comprehensive and reasonable diagnosis and treatment strategies according to available medical resources from the perspective of finding the etiology of EPD.
To compare the differences of hip offset and rotation center reconstruction between robot-assisted and manual total hip arthroplasty (THA).
Patients underwent robot-assisted and manual THA from May to September of 2020 in the First Affiliated Hospital of Chongqing Medical University were enrolled in this study. The patients included 27 patients (28 hips) in robot-assisted THA (rTHA) group and 29 patients (31 hips) in manual THA (mTHA) group. In rTHA group, there were 16 males and 11 females, with a mean age of (59±13) years. In mTHA group, there were 18 males and 11 females, with a mean age of (63±14) years. Basic information, including gender, age, body mass index (BMI), diagnosis and functional scoring etc, were recorded. In rTHA group, Mako robot system was used for preoperative planning, intraoperative real-time location and navigation. In mTHA group, traditional preoperative template design and surgical procedure were carried out. Operation time and functional scoring were compared postoperatively. Femoral offset, acetabular offset, global offset, rotation center changes in vertical and horizontal directions were measured on pelvis X-ray and analyzed. The correlation between intraoperative feedback of global offset change in robot system and postoperative measured global offset were analyzed.
Operation time in rTHA group was (80±10) min, which was statistically longer than that in mTHA group ((58±18) min, P<0.001). With 6 months′ follow-up, the Harris scoring in rTHA group was 94.9±2.8, which was statistically higher than that in mTHA group (93.1±2.8, P=0.017), however there was no statistic difference in WOMAC scoring between rTHA and mTHA group (7.0±3.8 vs 7.1±2.4, P=0.840). Absolute global offset change within 5 mm, 5-10 mm and lager than 10 mm were 71.4%(20/28), 28.6%(8/28) and 0 in rTHA group, which were 45.2%(14/31), 29.0%(9/31) and 25.8%(8/31) in mTHA group (all P<0.05). A positive relation was found between intraoperative feedback of global offset change in robot system and postoperative measured global offset in rTHA group (r=0.77, P<0.001). It was found that rotation center changes concentrated in outer upper quadrant in both groups, and rotation center change in rTHA group concentrated mainly in the area less than 10 mm, however, rotation center change in mTHA group was more dispersive compared with rTHA group.
rTHA may accurately reconstruct hip offset and rotation center, intraoperation feedback of global offset change may be an effective reference.
To assess the improvement of the robotic-assisted total hip arthroplasty (rTHA) regarding the acetabular cup positioning and the leg length restoration.
Clinical data of 246 patients undergoing primary unilateral THA from September 2019 to June 2021 in Peking University Third Hospital were retrospectively reviewed, including patients treated with rTHA (n=113) and conventional THA (cTHA) (n=133). Thirty-seven male patients along with 76 females were enrolled into the rTHA group with a mean age of (62±9) years. In comparison, the average age of cTHA group was (60±12) years with 59 males and 74 females. The postoperative inclination and anteversion of the acetabular cup and the rate of acetabular cup within the Lewinnek and Callanan "safe zone" were documented and analyzed. For the patient with normal or surgically restored contralateral hip, the leg length discrepancy was also measured and compared between the two groups.
The overall mean postoperative inclination and anteversion was 39.0°±5.5° and 14.1°±5.1°, respectively. No statistically significant difference was found between the rTHA and cTHA group regarding postoperative inclination (39.3°±2.9° vs 38.7°±7.0°, P=0.383) and anteversion (13.4°±4.3° vs 14.7°±5.6°, P=0.054). In rTHA group, 97.3%(110/113) of the cups were implanted within the Lewinnek"safe zone"(75.9%(101/133) in cTHA group) and 94.7%(107/113) were within the Callanan"safe zone"(66.2%(88/133) in cTHA group), and those were both higher in rTHA group (both P<0.01). There was no significant difference in postoperative leg length discrepancy between the two groups (P=0.445). Meanwhile, 29(23.0%) cases of cTHA group had leg length discrepancy more than 5 mm, and it was 9.7%(9/93) in rTHA group.
It indicated the benefit of rTHA in acetabular cup positioning as well as the leg length restoration.
To explore the learning curve and short-term clinical outcomes of Mako robotic-assisted direct anterior approach total hip arthroplasty (THA).
The preoperative basic data, surgical information and postoperative rehabilitation of 50 patients who underwent Mako robotic-assisted THA for hip diseases in Department of Orthopedic Surgery of the 6th People′s Hospital Affiliated to Shanghai Jiao Tong University from December 2018 to December 2020 were analyzed retrospectively, included operation time, intraoperative blood loss, postoperative complications, postoperative imaging parameters (abduction angle, anteversion angle, lower limb length difference, eccentricity difference) and postoperative hip joint Harris score (hip Harris score, HHS). There were 16 males and 34 females, with a mean age of 50-79(67±10) years. The postoperative clinical results of Mako robotic-assisted total hip arthroplasty was analyzed. A cumulative sum analysis (CUSUM) was performed on the operation time (OT). The CUSUM learning curve was modeled by curve fitting and R² was used to testify the goodness. The different phase of the learning curve was compared with several observation indicators.
All patients were followed up for more than 6 months. Two patients had poor wound healing and 5 patients had symptoms of lateral femoral cutaneous nerve injury, which disappeared within 1-2 months. No serious complications such as dislocation, aseptic loosening, periprosthetic infection or revision occurred in all the patients. The average operation time was (81±16) min, and the intraoperative blood loss was (456±84) ml. The average Harris hip score at the last follow-up was 88.6±2.5. The radiological evaluation showed that the positions of the acetabular cups were all in the Lewinnnek safety zone; the limb length discrepancy was (0.15±0.50) cm, the offset was (-0.11±0.72) cm. The OT decreased with the accumulation of the cases. The CUSUM learning curve was best modeled as cubic curve, the fitting curve reached the top at the 19th case. As a cut-off point, the 19th point divided the learing curve into two phases. There were statistical differences in OT, pelvic array installation time, acetabular registration time, acetabular reaming time (all P<0.05), but there was no significant differences in Harris hip score, acetabular prosthesis anteversion angle and abduction angle between the two groups (all P>0.05).
The learning curve of Mako robot-assisted DAA-THA is about 19 cases. Mako robot-assisted DAA-THA can ensure the accuracy of prosthetic placement and the safety of the operation during the learning curve, and the short-term clinical results after surgery is excellent.
To explore the differences of alignment and position of prosthesis between portable accelerometer-based navigation device (PAD) and conventional instrumentation (CI) in total knee arthroplasty (TKA) with valgus deformity.
Patients with knee osteoarthritis and valgus deformity who underwent primary TKA in Peking University Third Hospital from January 2017 to December 2020 were enrolled in this retrospective study and were divided into PAD group and CI group according to the surgical instruments. Five male patients and 44 female patients were included with a mean age of (67.2±7.0) years. The differences in preoperative general data, preoperative and postoperative alignment between the two groups were studied.
A total of 49 patients (25 patients in the PAD group and 24 in the CI group) were enrolled in this study. There were no statistically significant differences in gender, age, height, weight, body mass index, surgical side, preoperative hip-knee-ankle (HKA) angle, preoperative HKA angle deviation, Keblish classification and Ranawat classification between the two groups (all P>0.05). There was no significant difference in the accuracy of postoperative HKA angle (2.0°±1.4° vs 3.0°±2.2°, P=0.082), coronal femoral component angle (CFCA) (1.5°±1.2° vs 2.1°±1.6°, P=0.144) and coronal tibial component angle (CTCA) (1.2°±0.8° vs 1.3°±1.0°, P=0.695) between the two groups; but the standard deviation of the above-mentioned three indices in PAD group were all smaller than those in CI group. The rate of outliers of postoperative HKA angle of the PAD group was smaller than that in the CI group (P<0.05), but there was no significant difference in the rate of outliers of CFCA and CTCA between the two groups (both P>0.05).
TKA assisted by PAD can provide good alignment and prosthesis position in patients with valgus deformity, and it is superior to TKA with CI in terms of precision and rate of outliers of postoperative overall alignment of lower extremity.
To investigate the value of ischemia modified albumin (IMA) level for predicting in-hospital mortality in patients with acute aortic dissection (AAD).
A total of 195 patients with AAD from the Department of Cardio-Vascular Surgery of Affiliated Hospital of North Sichuan Medical College from January 2017 to November 2019 were consecutively collected, with 126 males and 69 females. Based on whether they died during hospitalization or not, these patients were divided into 2 groups: survival group and mortality group. The baseline data and IMA levels at admission of the two groups were recorded. Univariate logistic regression analysis was used to identify the independent risk factors, and multivariate logistic regression analysis was further performed on variables with statistical significance in univariate analysis. The area under the receiver operating characteristic (ROC) curve was calculated to determine the value of IMA for predicting in-hospital mortality in patients with AAD.
Forty-two AAD patients died and 153 survived, and the mortality rate was 21.5%. Logistic regression analysis showed that age (OR=2.143,95%CI:1.247-4.826,P=0.011), Stanford type A (OR=6.751,95%CI:3.189-14.291,P<0.001), drug therapy (OR=5.133,95%CI:2.463-10.700,P<0.001), IMA level (OR=4.452,95%CI:2.231-8.953,P=0.004) were independent risk factors for in-hospital mortality in patients with AAD, however surgery was a protective factor (OR=0.195,95%CI:0.093-0.406,P<0.001). The area under the ROC curve for IMA level in predicting in-hospital mortality with AAD was 0.838 (95%CI: 0.774-0.901, P<0.001), with a cut-off value of 86.55 U/ml, and the sensitivity and specificity were 83.3% and 75.2%, respectively.
IMA may serve as a simple risk assessment indicator for patients with AAD. IMA level at admission is an independent predictor of in-hospital mortality. For patients with higher IMA level, early surgical intervention should be performed.
To study the value of color doppler ultrasonography (CDU) in the diagnosis of endograft infections following endovascular aneurysm repair (EVAR).
The retrospective analysis of post-EVAR stent infections identified by computed tomography angiography (CTA) was conducted at the First Affiliated Hospital of Sun Yat-sen University from January 2010 to December 2020. There were 16 males and 4 females, aged from 49 to 86 years. All the patients were detected by CDU. The endoleak, bubbles, abscess, hematoma, aortic intestinal fistula (AEF) and occlusion of stent detected by CTA and CDU were analyzed and compared.
Among 20 patients, 9 cases with endoleak were detected by CTA, while CDU showed 8 cases with endoleak. The rate of missed diagnosis was 1/9. The misdiagnosis rate was 0, and the Youden index was 0.89. CDU detected 3 cases with type Ⅱ endoleak, and 1 case was missed when compared with CTA. Three cases with type Ⅰa and 2 cases with type Ⅰb were detected by CDU, which were consistent with those of CTA. CDU and CTA showed that there were no cases with type Ⅲ and type Ⅳ endoleaks. CDU detected 8 cases with bubbles in the sac. Compared with CTA, the rate of missed diagnosis was 2/10. The misdiagnosis rate was 0, and the Youden index was 0.80. The cases with abscess, hematoma, increasing size of the aneurysm, occlusion of stent and fluid sonolucent area in the sac detected by CDU were 8/20, 2/20, 4/20, 1/20, 2/20, which were consistent with CTA. CDU did not detect the 3 cases with aortoenteric fistula(AEF) which were identified by CTA. The follow-up of CDU showed that the extra-anatomic bypasses remained their patency in 5 cases, 1 case occurred bypass occlusion. The range of infectious area and bubbles reduced in 2 cases. There was no change of endoleak in 1 case. All the follow-up results were consistent with those of CTA.
CDU can comprehensively evaluate the infection in and around the aneurysm in patients with stentinfection after EVAR, with a high auuraly, and has important clinical significance for the early diagnosis, treatment and prognosis of patients.
To clarify the effect of high-frequency repetitive transcranial magnetic stimulation (rTMS) on cognitive function in cerebral ischemic rats, and to explore its underlying mechanism by RNA sequencing.
Thirty male Sprague-Dawley (SD) rats underwent transient middle cerebral artery occlusion (tMCAO). According to the Bederson score, 10 rats with a score of 1-3 were excluded, and the remaining 20 rats were then randomly divided into the tMCAO group (n=10) and the rTMS group (n=10). Meanwhile, 10 rats with sham operation were assigned to the sham group (n=10). Rats in the rTMS group received 20 Hz rTMS from day 7 to day 28 after surgery. From day 28 to day 33 after the operation, Morris water maze test was performed to detect the cognitive function of rats in each group. The cortical tissues around the infarcts from the rTMS tMCAO groups were taken for RNA sequencing analysis, with 3 rats in each group.
The escape latency of rats in the rTMS group[ (53±4)s] and the group [(51±5)s] were significantly shorter than that of the tMCAO group[ (58±4)s, P<0.05)]. The times that the rats crossed the original platform in 60 seconds in the rTMS group[2.5 (1.5-3.0)] and sham group[3.0 (1.5-3.0)] were more than that of the tMCAO group [1.0(0.5-1.5)] (P<0.05). RNA sequencing detected 16 significantly differentially expressed genes, including 9 up-regulated genes and 7 down-regulated genes. GO analysis showed that the functions of up-regulated genes were mainly concentrated in the processes of chemical homeostasis and cell metal ion homeostasis. While the functions of down-regulated genes mainly enriched in the inflammatory response.
Twenty Hz rTMS can improve the cognitive function of rats with cerebral infarction, and its underlying mechanism may be related to maintaining chemical and metal ion homeostasis and regulating the polarization of microglia to reduce neuroinflammation.
This article reports a case of chronic tophic arthritis treated with labulihai. The patient was a 30-year-old young man with a gout course of 12 years and a tophi course of 6 years. He could not tolerate the maximum dose of febuxostat combined with benzbromarone in the past. After admission, he had multiple large tophi throughout his body. The serum uric acid was 627 μ mol/L, and the total urate volume of both hands, knees and feet was 167.91 cm3, on the basis of regular oral administration of febuxostat 80 mg/d and benzbromarone 50 mg/d, with the patient's informed consent, the blood uric acid was reduced to a minimum of 186 μ mol/L during treatment, the volume of tophi was reduced after 3 months of treatment, and the volume of urate in dual-energy CT was reduced by 33.6%. Labulihai is well tolerated and warrants further investigation.
The placement of the prosthesis is one of the most important factors affecting the long-term clinical outcome after total knee arthroplasty (TKA). In order to improve prosthesis placement, personalized osteotomy guides (PSIs) based on patient MRI or CT data have been used in recent years to assist TKA. However, there are still controversies about the accuracy of PSI, especially in the placement of tibial prostheses, with mixed conclusions.
Over the past decades, total knee arthroplasty (TKA) has been recognized as an effective solution to reduce pain and restore their joint function in patients such as severe osteoarthritis, so TKA has become increasingly common. However, about 20% of patients are still unsatisfied after TKA. The accuracy of prosthesis placement is still considered to be one of the key factors to improve patient clinical function after surgery and improve patient satisfaction after surgery. Among them, computer navigation assisted (CAS) TKA is considered to assist clinicians in more accurate prosthesis placement. However, current studies on the evaluation of the effect of CAS application in TKA have mixed conclusions and the follow-up time is too short. Therefore, the aim of this study was to compare the differences between CAS-assisted TKA and traditional TKA in perioperative conditions, postoperative clinical functional conditions and prosthetic imaging measures.
