Postherpetic neuralgia (PHN), defined as pain lasting ≥1 month after the healing of herpes zoster rash, is the most common complication of herpes zoster. Patients are often accompanied by impairment of emotion, sleep and quality of life, especially patients with stubborn PHN who have unsatisfactory results in various treatment methods, often have severe negative emotions and depressive symptoms. Common treatment methods for PHN include conventional oral pain medications and nerve block therapy, but only provide temporary pain relief. Pulsed radio frequency modulation (PRF) can improve the patient's condition, but the efficacy is not long-lasting. In this study, 36 patients with refractory upper thoracic PHN underwent CT-guided radiofrequency thermocoagulation (RFT) under conscious sedation. The Pain Numerical Rating Scale (NRS) was recorded before and 2 h, 1 week, 4 weeks and 8 weeks after surgery, and the mental state of patients was assessed by SF-36 clinical questionnaire before and 4 weeks and 8 weeks after surgery. The results showed that after RFT treatment, the NRS scores were significantly decreased and the SF-36 scores were significantly increased. The results show that CT-guided lower intervertebral foraminal puncture (RFT) is effective in the treatment of refractory upper thoracic PHN, and can effectively improve the depression and quality of life of patients.
