Corneal refractive surgery,especially excimer laser corneal refractive surgery,has been used nearly 20 years in China.As the development of high technology and the change of life style,people who received refractive surgery not only demands good visual acuity but also good visual quality.In recent years,more and more new techniques and equipments have sprung up in this field.Therefore,this provides more approaches for surgeon to design the surgical plan and meet patients'needs to see clearly and comfortably.In this article,the author minutely dissects the importance and influence of the design procedure from the perspective of scotopic vision (such as:design of thin cornea,severe astigmatism,scotopic large pupil size,and special corneal shape) and personalized surgical parameters.

Objective To investigate changes of visual performance and effects of higher order aberration on visual performance after Epi-LASIK for myopia.Method In this prospective study,102 patients were included.The trial group had 52 patients with Epi-LASIK,and 50 cases with LASIK as the control group.Ocular higher order aberrations were measured with the Hartman-shack wavefront analyzer,and high (100%),moderate(25%),low contrast(10%) and very low contrast(5%)visual acuity with and without glare were measured with Multi-Function Visual Acuity Tester under mesopic condition preoperatively and 4-6 months after surgery.35 subjects in Epi-LASIK and 33 cases in LASIK were followed up 4-6 months later.Paired t test were used to analyze different contrast visual acuity before and after Epi-LASIk. Multiple linear regression analysis Was performed to find the explanatory aberrations which contributed to the visual acuity.The Pearson correlation was used to explore the correlation between high-and low -contrast visual acuity and the explanatory higher order aberrations.The independent t test was used to analyze the difference of refraction power and visual acuity between Epi-LASIK and LASIK.Results There were significantly statistical differences at 5%and 10%contrast Ievel compared to preoperation with glare and with no glare.With no glare,the mean IogMAR visual acuity at 5%and 1O%contrast level were 0.32±0.14 and 0.21±0.12 preoperatively,and 0.47±0.15 and 0.33±0.16 postoperatively(t=-4.863,3.950;P=0.000,0.000).With glare,the mean logMAR visual acuity were 0.37±0.20 and 0.22±0.13 preoperatively,and 0.46±0.18 and 0.30±0.17 postoperatively(t=-1.949,-2.283:P=0.047,0.029).Using stepwise multiple linear regressions to evaluate which higher order aberration was significantly associated with each contrast VA. For each order RMS.S_4 was positive correlated with 5% contrast visual acuity(r~2=0.282;P=0.025);S_4 was positive and S_4 was negative correlated with 10% contrast visual acuity(r~2=0.192;P=0.033).None of the higher order RMS can significantly affect the 25% and 100% contrast visual acuity.For each Zernike coefficients,C_4~0 and C_6~4 were positive correlated with 5% contrast visual acuity(r~2=0.233;P=0.024);C_4~(-4) was negative,C_4~0 and C_5~(-5) were positive correlated with 10% contrast visual acuity(r~2=0.289;P=0.013);None of Zernike coefficients had a significant correlation with 25% and 100% contrast visual acuity.The Pearson correlated coefficients between S_4,C_4~0 and each contrast visual acuity increased as the contrast level decreased.S_4 and C_4~0 were associated significantly with 5% contrast visual acuity,the correlation coefficients were 0.378 and 0.390(P=0.025 and 0.021).The visual acuity under 100% contrast with no glare was a little poorer than in LASIK.Conclusions At the early stage after Epi-LASIK.the visual acuity for lower contrast level was worse compared to preoperative with glare and with no glare.Spherical aberration and the spherical-like aberration may contribute to this.

Objective To assess and compare the accuracy and regularity and short-term change of corneal flap in different flap preparation styles.Methods Eighty eyes of 40 cases were treated with laser-assisted subepithelial keratectomy.One hundred and fifty-five eyes of 78 cases were treated with laser in situ keratomileusis with 90 μm microkeratome flap preparation and eighty-two eyes of 42 cases were treated with laser in situ keratomileusis with 100 μm femtosecond laser flap preparation.Anterior segment optical coherence tomography Was used to assess the thickness of epithelial or stromal flaps created with M2 microkeratome or VisuMaX femtosecond laser by the same investigator 1 hour,1 day,1 week,1 month and3 months postoperatively,respectively.The thickness of twenty-five measuring points across each flap in LASIK,which were 1.5mm,2.5 mm and 3.5 mm away from vertex of cornea on the horizontal,vertical,45°and 135°meridian respectively and the vertex of cornea itself,Was evaluated.The results were analyzed with t-test or ANOVA by SPSS software.Results The thickness of epithelial flaps Was(57.91±6.76),(54.94±4.65),(54.96±2.70),(55.26±4.27)and(53.86±5.56)μm at 1 hour,1 day,1 week, 1 month and 3 months respectively,with statistical significant difference between 1 hour and the other time points(F=20.25,P<0.05).The average thickness of stromal flaps on the vertex of cornea in microkeratome group was (129.16 ±12.10)μm,while(112.69±5.12)μm in the femtosecond laser group,which means the accuracy of flap preparation with femtosecond laser is better than with microkeratome.The absolute difference between actual and average thickness was (13.16±4.78)and (5.26±1.21)μm in the microkeratome group and femtosecond laser group,which means the reproducibility of flap preparation with femtosecond laser is better than with microkeratome as well. Flap morphology showed the feature of thin central area and thick peripheral area in microkeratome flap and uniformity in femtosecond laser flap.which illustrated the better regularity of femtosecond laser flap preparation.Conclusions Anterior segment optical coherence tomography is a safe.quick and reliable facility to examine and assess flap morphology.Our results showed the accuracy and regularity of femtosecond laser flap preparation outweighed microkeratome flap preparation.

：Objective To evaluatethe changes of corneal biomechanical properties and corneal topography undergoingovernight orthokeratology treatment. Methods Thirty teenagers with low andmoderate myopiawith age of 11.67±1.63,myopia(-2.56±0.86)D,were included and were fitted with Ortho-KCL. The corneal biomechanical properties,including corneal hysteresis (CH),cornealresistance factor(CRF),goldmann-correlated lOP(IOPg) and corneal-compensated intraocularpressure(IOPcc) were measured with ocular response analyzer(ORA).Cornealtopography,central corneal thickness(CCT) and corneal endothelium density were measuredwith computerized corneal topography,optical coherence tomography(OCT) and non contactspecular microscope respectively.The measurements were taken at pre-wear,1 day and7,30,90,180 days after orthokeratology.Only data from the right eyes were analysed.ResultsAll subjects were significantly reduced the myopia amount after 1 day of lens Wear.Thecorneal curvature flattening(F=38.837,P<0.01)became slightly down to stable after thefirst week.There were significant decrease in CH and CRF after the orthokeratologytreatment within the first week,and CH and CRF reversed and thereafeter into the originallevel at 3-month and 6-month follow up.IOPg and IOPcc decreased and reached the lowwestlevel at 1-week visit and after then became down to stable.There were significantreduction in CCT after 1 week(F=4.739,P<0.05).There were no significant changes incorneal endothelium density during orthokeratology treatment for 6 months.Conclusions Theamount of myopia reduction with orthokeratology occurred mostly within 1 week while thecorneal biomechanical properties such as CH and CRF were decreased. However the cornealbiomechanical properties are reversal to the original level thereafter and remainunchanged within the 6 months follow up visits.It proves that orthokeratology does notdemage corneal microsturcture.The early sign of reduction may due to the temporal responseto the reshaping of the cornea.

Objective To study how the cornea would express Ki-67,TGF-β_2 and α-SMA and how the cornea would heal when it is scanned intrastromally.Methods Twenty New Zealand white rabbits Were randomly numbered and used for the experiments. The 60 kHz Intralase femtosecond laser delivery system was used to scan 35 rabbit corneas (of these corneas,5 right corneas were from 5 rabbits).The unoperated 5 left eyes were used as control.The laser settings were;spot/line separation,10μm;diameter,8.5mm;energy for scarnning the stroma,1.3μJ;scanning depth,135 μm,no edge cuts were performed.Slit lamp was used to observe the cornea at 0.5 h,1 h,2 h,and 3 months after surgery.On day 1,3,7,30,90 after surgery, 6 and 1 scanned corneas at each time point were taken for Western blot and immunocytochemical detection respectively to detect the expression of,TNF-α,Ki-67,TGF-β_2 and α-SMA.Results Microbubbles were found in crneal stroma after surgery.The number of microbubbles tended to decrease significantly with time.At 2 h.the cornea turned to be transparent again.During 3-month follow-up,the cornea was always transparent and no haze appeared.Western blot analysis and immunocytochemical detection demonstrated that the expression of Ki-67 began to increase at day 1(0.0670±0.0008)and reached the peak at day 3 after surgery(0.6923±0.005).While there were significant difference with the control group at day 90 after surgery(t=24.12,57.22,43.26,39.78,18.35;P<0.05),a gradual decrease in expression of ki-67 could be detected from day 3 to 90.There were no significant change in the expression of TGF-β_2 (t=0.933,0.856,0.934,0.970,1.132)and α-SMA (t=1.126,1.235,0.993,1.175,1.211)at all time points after surgery(P>0.05).Conclusions After intrastromal scanning,the expression of Ki-67 increases and the keratocyte is activated and proliferated.However,due to no epithelium injured and TGF-β_2 confined to the epithelium,the activated and proliferated keratocytes could not be transformed into myofibroblasts.

Objective To study effects of vitreous injecting M_1-selective muscarinic antagonist,pirenzepine,on expression of M_1 and M_4 receptor in retina,choroid,schera and iris-ciliary body of guinea pig with form-deprived myopia.Methods Twenty-four 1-2 week-old pigmented guinea pigs were randomly divided into four groups.Groupl:normal control (N) (n=6);group 2:simple form-deprived myopia (FDM)(n=6);group 3:drug control (S) (n=6);group 4:pirenzepine(P) (n=6).Expression changes of M_1 and M_4 muscarinic receptors at mRNA level were detected by semi-quantitative RT-PCR in retina,choroid,sclera and iris-ciliary body.Result 1.After 21 days'treatment,FDM group produced relative myopia of -4.92 D and an axial length of 0.29 mm with significance(P<0.001),compared with N group.Compared with group S,the relative refractive error in group P wag+0.88 D,and axial length decreased 0.30 mm with respectively significance(P<0.001).Differences in refractive error between grouP S and group FDM were not significant,but in axial length were significant(0.08mm vs.0.29mm)(P<0.05).2.Semi-quantitative RT-PCR:M_1 and M_4 mRNA expression showed no significant differences in the retina,choroid and iris-ciliary body of group P compared with group S(P>0.05).Of interest,in the posterior sclera,mRNA expression of group P was significantly greater than that of group S for the M_1(P< 0.05)and M_4 subtypes(P<0.05).The M_1 and M_4 subtype in group P was increased by +19.16%and +64.29% respectively. Conclusion M_1-selective muscarinic antagonist,pirenzepine,can effectively inhibit form-deprived myopia in guinea pig. M_1 and M_4 subtype in sclera and their cholinergic signaling may participate in muscarinic antagonist inhibition of myopic development.

Objective To explore the influences on modulation transfer function(MTF) as a function of age and pupil in adult emmetropic eyes.Methods It was a prospective clinical study.All the 102 emmetropic eyes were divided into three groups according age:group A,40 eyes (age 20-30 years);group B,30 eyes (age 31-40 years);group C,30 eyes (age 41-50 years).The total MTF and subtracted lower order aberration MTF curve were measured in all emmetropic eyes in 3.0mm.4.0 mm and 5.0 mm pupil sizes with a iTrace Visual Function Analyzer at six spatial frequencies(5,10,15,20,25,30 cpd) .The associateion between age and the two MTFs was analyzed,the two MTFs were compared among different age groups and different pupils using univariate analysis.Results In 3.0 mm,4.0 mm and 5.0 mm pupil sizes and at all six spatial frequencies,there was a negative correlation between age and the two MTFs,there were significant differences among the three groups(F=18.773,P<0.05).At all six spatial frequencies,the two MTFs decreased with the increasing pupil. Conclusions The total MTF and subtracted lower order aberration MTF in adult emmetropic eyes decreased with aging and increasing pupil,so when using the two MTFs to evaluate the visual quality of human eyes,We must use different criteria according to the different age and pupil size.

Objective To investigate the feasibility,efficacy,and complications of the temporary anchor sutures for endothelial graft fixation in Descemet stripping endothelial keratoplasty (DSEK) in aphakic eyes.Methods Non-randomized retrospective case series.Twelve consecutive cases (12 eyes)with aphakic bullous keratopathy underwent DSEK using the temporary anchor sutures technique for fixation of donor button in Zhongshan Ophthalmic Center between April to December in 2007.All cases were resulted from lens extraction and best spectacle-corrected visual acuity (BSCVA) was inferior or equal to 0.04.Eight cases were with preoperative pain. Prospective data of BSCVA,corneal astigmatism,curvature and thickness were collected on all 12 eyes preoperatively and at 3,6 and 12 months postoperatively.Average endothelial cell density (ECD) and endothelial cell loss (ECL) were recorded at 6 and 12 months postoperatively.The follow-up period was 12 to 20 months.Results All cases were performed DSEK successfully using the temporary anchor sutures and the grafts were well-attached without dislocation.On postoperative day 1,papillary block glaucoma occurred in 2 eyes and mannitol 20% was administered intravenously.The intraocular pressure in the 2 eyes returned to normal due to the air burble absorption on postoperative day 2.The endothelial grafts showed mild to moderate edema in all eyes and became clear in 8 to 15 days postoperatively.all the corneas were clear during follow-up.In 12 months postoperatively,BSCVA was 0.7 in 16.7% of eyes (n=2),0.5 in 25.0%of eyes(n=3),0.4 in 33.3% of eyes(n=4),and 0.3 in 25.0%of eyes(n=3);mean corneal astigmatism was(2.40±0.70) diopters,mean corneal curvature was (45.40±1.50) diopters,and mean corneal thickness was (591.5±20.4) μm. Mean endothelial cell density (ECD) was (2088±146)cells/mm~2[(28.9±3.9)% endothelial cell loss] and (1857±101)cells/mm~2[(36.7±3.1)% endothelial cell loss] in 6 and 12 months postoperatively,respectively.Conclusion The temporary anchor sutures technique was easy to perform and effectively prevented graft dislocation and detachment into the vitreous cavity.

Objective To investigate the feasibility and clinical effect of non-Descemet stripping endothelial keratoplasty for bullous keratopathy.Methods In the prospective consecutive case series,6 patients (6 eyes) with bullous keratopathy at Affiliated Xiamen Eye Center of Xiamen University from September 2007 to January 2009 underwent non-Descemet stripping endothelial keratoplasty.During the surgical procedures.the recipient's Descemet membrane and endothelial layer were left untreated.The donor tissue disc consisting of posterior stroma and healthY endothelium from a donor cornea was implanted into the anterior chamber from a limbal tunnel incision,then unfolded and pressed against the recipient cornea with sterile air filled in anterior chamber.All patients were followed up for 6-20 months,pre-and postoperative best corrected visual acuity (BCVA) were compared,the adherence of the donor disc to the recipient endothelium and postoperative donor disc dislocation were monitored during the follow-uD period.Graft clearance and endothelial cell density (ECD) were observed,too.Results Five donor disc were well adhered to the recipient endothelium and no graft dislocation occurred during the follow-up period.One case showed a narrow gap between the host endothelium and donor disc 1 day postoperatively.This patient was told to stay in prone position and the donor disc readhered well.All the grafts remained transparent until the latest follow-up.Five patients had improved BCVA while 1 patient had the same BCVA.The reason for poor VA was optic atrophy due to trauma.Postoperative mean EDC was (1648±384) cells/mm~2.Conclusions Non-Descemet stripping endothelial keratoplasty for bullous keratopathy is feasible,technically easy,safe and effective.It can be one of the surgical treatment option for bullous keratopathy.

Objective To investigate whether there are some differences in the accuracy of Acrysof Toric intraocular lens(IOL)implantation and the ability to correct astigmatism between 2 types of clear Corneal incisions used in phacoemulsification:the on-axis (I.e on the steeper corneal meridian) and the temporal clear corneal incision (OCCI and TCCI,respectively).Methods In this prospective study,thirty-nine eyes with cataract and mild to moderate corneal astigmatism (1.00-2.26 diopters on corneal curvature) were recruited and divided into OCCI group (20 eyes) and TCCI group (19 eyes) respectively.All of these patients underwent phacoemulsification and an Acrysof Toric IOL implantation (single surgeon,3.2 mm incision).Main outcome measurements included uncorrected visual acuity (UCVA),best corrected visual acuity (BCVA),corneal curvature,surgically induced astigmatism (SIA),deviation of lens axis(LAD),index of corrected astigmatism (CAI) and accurace of astigmatism correction(ACA) at 3 months postoperatively.Results At 3 months postoperatively,simple algebraic difference showed a mean increase in the astigmatism magnitude of (0.28±0.27) D in the TCCI group and a mean reduction in the astigmatism magnitude of (0.31±0.26)D in the OCCI group(t=5.756,F=0.000).The average LAD wag 0°±0°in the OCCI group and 2.22°±2.99°in the TCCI group(t=3.860,P=0.000).The average CAI were 1.04 ±0.27 and 0.59±0.44 respectively (t=3.187,P=0.004).The ACA were 0.838 and 0.484 respectively (Z=1.970,P=0.049).Conclusions Compared with the TCCI,the OCCI could correct some corneal astigmatism and reduce IAD induced by the SIA shift. As a result,the OCCI could improve the accuracy of Acrysof Toric lens implantation and the ability to correct astigmatism.

Objective To study the value of Pentacam in the diagnosis and localization of intralenticular foreign body.Methods It was a comparative study.In this retrospective study,9 cases with intralenticular foreign bodies were examined by using slit-lamp, B-mode ultrasound,ultrasound biomicroscopy (UBM),CT and Pentacam.The results were confirmed by operation.Results Foreign bodies in 4 cases were detected by slit-lamp,with 1 case suspected;5 cases by B-mode ultrasound;and 6 cases by UBM, CT and Pentacam.Projectile foreign bodies penetrated cornea and were remained in the lens in all 9 cases.In all cases,Pentacam detected all related signs of intralenticular foreign body,including corneal wound of entry,wound or organized wound in the anterior lens capsule,bulge of cortex lentis and traumatic cataract.Pentacam also could localize the meridian of foreign body precisely and could recognize the relationship between foreign body and neighour tissues.But it Was difficult to recognize the nature of foreign body.The measured size of foreign bodies in Pentacam image was a little smaller than the actual size.Conclusions Pentacam has a valuable role in the diagnosis of intralenficular foreign body and the related damages.Pentacam could indicate precise location of intralenticular foreign body.Combined with multiple image-related methods.Pentacam can provide accurate and comprehensive information in the diagnosis and localization of intralenticular foreign body.

Congenital cataract is the common cause of visual disability in children.Inherited isolated(non-syndromic) cataract represents one third of cases.Currently,at least 22 specific genes associated with isolated inherited cataract have been identified:ten crystallin genes:CRYAA,CRYAB,CRYBA1/A3, CRYBA4,CRYBB1,CRYBB2,CRYBB3, CRYGC,CRYGD,CRYGS;4 membrane protein genes:GJA3,GJA8,MIP,LIM2;three growth and transcription factor genes:PITX3,MAF,HSF4;two cytoskeletal protein gene:BSFP1,BSFP2;chromatin modifying protein-4B gene:CHMP4B,EPHA2 and NHS,it is likely that more genes remain to be discovered.Some of the genes have been studied for their function by expression in cells or/and by knock-out animal models.The increasing availability of more detailed information about their functions makes it possible to understand the pathophysiology of congenital cataracts.

Wavefront aberration technique is an important objective criterion to evaluate the optical quality of human eye.Higher order wavefront aberrations and accommodation affect each other,influencing retinal image quality of the human eye which is a dynamic optic system.Investigation of the interaction between these two factors helps in revealing reasonable explanation for visual symptoms.This paper reviewed higher order aberrations related points such as their clue for accommodation,changes related withaccommodation, microfluctuation during accommodative relaxation,and accommodation related higher order aberrations changes in myopia.

The mechanism of corneal neovascularization after alkali burn and its drug therapy have been the focus of research in the field of ophthalmology. Recent studies have shown that hepatocyte growth factor (HGF) is a strong pro-angiogenesis factor ~ ([1]), and low-molecular-weight heparin (LMWH) can inhibit neovascularization ~ ([2]). However, the relationship between HGF and corneal neovascularization and the inhibition of corneal neovascularization by LMWH are rarely reported at home and abroad. In this experiment, a rat corneal neovascularization model was prepared by alkali burn, and the protein and mRNA expressions of HGF and vascular endothelial growth factor (VEGF) at different times after alkali burn were measured. The inhibitory effect of LMWH on corneal neovascularization was observed, and the mechanism of local application of LMWH on corneal neovascularization was discussed.

A 72-year-old male patient came to the Ophthalmology Center of Tianjin Medical University in December 2008 due to gradual blurred vision in his right eye for 1 week. The patient underwent phacoemulsification and cataract aspiration combined with intraocular lens (IOL) implantation in his right eye in 2002. The best corrected visual acuity was 0.3.

In recent years, the concept of cataract refractive surgery has penetrated into the clinical diagnosis and treatment of cataract diseases. It is the higher pursuit of cataract surgeons to restore patients' vision like young people. Multifocal intraocular lens has attracted more and more attention of ophthalmologists because of its good clinical application effect. The aspherical AcrySof ReSTOR +3.00D intraocular lens, which entered the clinic in 2009, can provide better far, medium and near vision for patients after cataract surgery.

A 26-year-old female patient came to the Department of Ophthalmology of Eye, Ear, Nose and Throat Hospital of Fudan University in October 2006 due to high myopia in both eyes and planned to undergo excimer laser surgery.

retinal artery occlusion (RAO) mostly occurs in the elderly with hypertension, diabetes, heart disease and carotid atherosclerosis ([1]).

Primary retinitis pigmentosa (RP) is a progressive hereditary dystrophic degenerative disease with a prevalence of about 1/4000, which is not a common disease, while sector retinitis pigmentosa (sector RP) is relatively rare. In December 2008, a patient with binocular sector RP combined with binocular acute angle closure glaucoma was treated in our hospital, which is extremely rare clinically. The report is as follows.

The patient, a 7-year-old female, came to the Department of Ophthalmology of Cangzhou Central Hospital of Hebei Province on August 6, 2009 due to low vision accompanied by nystagmus and strabismus since childhood. Her mother suffered from congenital cataract, nystagmus and strabismus in both eyes, and retinal detachment in her left eye.

Conjunctivalochalasis is a common age-related ocular surface disease in the elderly. With the accelerating aging of the population, the number of people with conjunctiva relaxation will increase day by day. Severe conjunctival relaxation requires surgical treatment. In order to standardize the surgical treatment plan and prevent iatrogenic ocular surface damage, Professor Zhang Xingru combined his more than 10 years' research results on surgical treatment of this disease (including key technologies such as indications for surgical treatment, standardized surgical operation, prevention and treatment of surgical complications) with literature and materials to make a CD.

Chinese Journal of Ophthalmology was founded on October 1, 1950. In the course of running the journal for nearly 60 years, there have been many twists and turns. From 1960 to 1964, due to natural disasters in the country and paper difficulties, it was suspended for the first time; From 1966 to 1978, it was forced to cease publication again due to the unrest until it resumed in the autumn of 1978. After more than half a century of ups and downs, several generations of ophthalmologists have paid hard work and sweat, and made the Chinese Journal of Ophthalmology the leading fine academic journal in China's ophthalmology community, which represents the national level externally and guides academic development internally.

It is estimated that by 2010, the number of glaucoma patients in the world will exceed 60 million, and the number of blind people will be nearly 10 million. The World Health Organization (WHO) has made the prevention and treatment of primary glaucoma one of the key tasks of blindness prevention. On the occasion of "World Glaucoma Day" on March 12th, 2010, in order to advocate the majority of ophthalmologists to pay attention to the prevention and treatment of glaucoma, Chinese Journal of Ophthalmology of Chinese Medical Association and Alcon (China) Ophthalmic Products Co., Ltd. jointly held the 2010 Alcon "Healthy Intraocular Pressure" Cup prize-winning essay competition.
