randomized controlled trial (RCT) is the core method to evaluate the effectiveness and safety of medical intervention. However, in pediatric research, traditional RCT often faces the challenges of limited sample size, strict ethical requirements and significant individual differences. Children are highly heterogeneous in their physiological development, making "average efficacy" difficult to guide individual treatment decisions. As an innovative design, nested-precision RCT (npRCT) provides a scientific, efficient and ethical solution for pediatric precision treatment research. npRCT allows simultaneous prospective validation of traditional efficacy evaluation and precise allocation strategies within the same research framework, thereby improving the clinical translatability of evidence.
