Acute gastrointestinal hemorrhage is one of the common medical emergencies in clinic. Implementing rapid and effective diagnosis and treatment for patients at the first time is the key to saving patients' lives and improving their prognosis. Therefore, in clinical work, it is a clinical skill that every physician should master to quickly judge the cause of gastrointestinal bleeding, accurately assess the severity of the disease, and take appropriate measures for different situations.
stress ulcer (SU) refers to acute gastrointestinal mucosal erosion, ulcer and other lesions that occur in various stressful states such as severe trauma, critical illness or severe mental illness. In severe cases, it can be complicated by gastrointestinal bleeding or even perforation, which can aggravate and worsen the original disease and increase the mortality rate. Therefore, prevention of SU is a link that cannot be ignored in the treatment of critically ill patients. SU can be manifested as acute gastric mucosal lesions, acute erosive gastritis, acute hemorrhagic gastritis, peptic ulcer, etc. under endoscopy.
A 51-year-old female was admitted to the hospital mainly for "anorexia, fatigue for 6 months and fever in the afternoon for 20 days". The patient developed anorexia, general fatigue and weight loss of approximately 4 kg without obvious trigger 6 months ago. Afternoon fever occurred 20 days ago, with a maximum body temperature of 39 ℃. There was no fear of cold, night sweats, no cough, expectoration and hemoptysis, no nausea, vomiting, no abdominal pain, abdominal distension and diarrhea, and the body temperature could drop to normal after symptomatic treatment. Abdominal B-ultrasound was performed in other hospitals to find multiple liver mass occupation, and multiple liver metastases were considered. An ultrasound-guided liver biopsy was performed in an external hospital, and the result was chronic granulomatous inflammation of the liver, showing caseous necrosis, and no acid-fast bacilli were found in acid-fast staining. Considering inflammatory lesions, neoplastic lesions were not excluded. 30 years of prior chronic viral hepatitis B (hepatitis B) history. Physical examination showed no obvious positive signs.
inflammatory bowel disease (IBD) is a chronic recurrent disease of the gastrointestinal tract, mainly including ulcerative colitis (UC) and Crohn's disease (CD).
