Diabetic retinopathy is one of the three major microvascular complications of diabetes. Proliferative diabetic retinopathy (PDR) is an important ocular complication that affects the vision and quality of life of patients. Vitrectomy is the most effective treatment option. The characteristic of fibrovascular membranes in PDR varies at different stages of the disease, which requires specific treatment strategy and techniques. In order to improve the clinical diagnosis and treatment of PDR, this article proposes that different treatment methods should be correctly adopted for different types of proliferative membranes on the basis of fully understanding the tissue morphology of these membranes, and more attention should be paid to the treatment of the vitreous body to achieve more effective therapeutic effects. Immunologic diseases is one of the important causes of dry eye. Dry eye related to immunological diseases, with more severe symptoms and signs and even vision loss, has complicated pathogenesis and is difficult to be diagnosed and treated. Thus, in order to improve the awareness among ophthalmologists, experts from the Chinese Branch of the Asian Dry Eye Society and other related academic organizations have conducted indepth discussions on the dry eye related to immunologic diseases, in terms of its definition, pathogenesis, clinical characteristics, diagnosis and treatment based on the clinical experience and related researches in this field in our country as well as international research progress, to form expert consensus opinions for reference in clinical work. This consensus further standardizes and promotes the diagnosis and treatment of dry eye related to immunologic diseases in China. (Chin J Ophthalmol, 2021, 57: 898-907)
Objective To study the long-term changes of the morphological parameters of the posterior pole in highly myopic patients over 40 years old in Chinese natural population. Methods A population-based follow-up study. The study included 71 patients (27 males, 44 females; 110 eyes) with high myopia who completed the Beijing Eye Study in 2001 and 2011 and had assessable color fundus photographs. The age of the patients in 2011 was 56.2±9.5 years old. According to the definition of pathological myopia by the meta-analysis of the Pathological Myopia Research Group, the fundus photos in 2001 were interpreted, and 110 eyes were divided into groups of simple high myopia (79 eyes) and pathological myopia (31 eyes). The morphological parameters during the 2 follow-up visits in 2001 and 2011 were measured in the color fundus photos, including optic disc tilt ratio, optic disc rotation degree, β zone area, disc-fovea distance, disc-outer β zone border distance, fovea-outer β zone border distance, and vertical distance between temporal arterial arcades, and compared in all the subjects, patients with simple high myopia and patients with pathological myopia. Correlations between the 10-year changes of these parameters and other parameters were analyzed using multiple linear regression analysis. Results In all subjects, the incidence of optic disc tilt and rotation was 41.1% (44/107) and 63.3% (68/107), respectively. The optic disc tilt ratio in 2001 and 2011 was 1.34±0.31 and 1.34±0.33, respectively. The optic disc rotation degree was 30.4±26.3° and 30.0±25.3 in 2001 and 2011, respectively. The differences in optic disc tilt ratio (Z=-0.317, P>0.05) and optic disc rotation degree (t=0.159, P>0.05) were not statistically significant. The β zone area [1.34 (0.45,3.73)mm2, Z=-7.295, P<0.05], disc-fovea distance [(5.18±0.54)mm,t=-3.523, P<0.05], and disc-outer β zone border distance [1.55 (0.24,2.16)mm,Z=-6.825, P<0.05] in 2011 were significantly increased compared to those in 2001 [1.05 (0.23,1.88)mm2, 5.08±0.43, 1.39 (0.77,1.93)mm]. There was no statistically significant difference in the vertical distance between temporal arterial arcades (Z=-1.858, P>0.05). The fovea-outer β zone border distance in the simple high myopia group in 2011 was significantly shorter than that in 2001 (Z=-3.221, P<0.05), while there was no significant change in this parameter in the pathological myopia group (t=0.655, P<0.05). In multivariate analysis, larger β zone area was significantly correlated with longer corneal curvature (r=0.462, P<0.05) and longer axial length (P<0.05). Longer disc-fovea distance was significantly correlated with lower best corrected visual acuity (r=0.348, P<0.05) and longer axial length (P<0.05). Similarly, longer disc-outer β zone border distance was related to lower best corrected visual acuity (r=0.538, P<0.05) and longer axial length (P<0.05). In addition, the progression of myopic maculopathy (Z=-3.425, P<0.05) and the progression from simple high myopia to pathological myopia (Z=-2.911, P<0.05) were both related to enlargement of the β zone. Conclusion For patients with high myopia aged over 40 years, the optic disc morphology is relatively stable, while the disc-fovea distance and β zone area will continue to increase with the progression of myopia. This may be due to the elongation of axial length. Enlargement of the β zone may be a predictor for the progression of myopic maculopathy.(Chin J Ophthalmol, 2021, 57:908-915)
To explore the application and value of iris angiography combined with ultra-wide-field fundus fluorescein angiography (UWFA) in diabetic retinopathy (DR).
This was a cross-sectional study. From May 2016 to December 2019, 60 consecutive DR patients (120 eyes) including 30 patients (60 eyes) with severe non-proliferative DR (NPDR) and proliferative DR (PDR), respectively, who underwent iris angiography combined with UWFA in Tianjin Medical University Eye Hospital were enrolled in the study. There were 25 males and 35 females, with an average age of (53.5±10.7) years. All of the patients underwent ophthalmologic examination including visual acuity, intraocular pressure, slit-lamp microscopy, slit-lamp funduscopy, ultra-wide-field fundus photography, and iris angiography combined with UWFA. Iris angiography included iris fluorescein angiography (IFA) and iris indocyanine green angiography. The onset time of the iris vascular fluorescein leakage was recorded, and the circumference range of the pupil margin fluorescein leakage was measured by a self-developed software. Wilcoxon rank sum test was used to compare and analyze the difference in the onset time and the range of the iris vascular fluorescein leakage between the severe NPDR and PDR groups.
IFA showed that the onset time of the iris vascular fluorescein leakage was 31.00 (25.75, 34.00) s and 27.00 (21.75, 29.50) s in the severe NPDR group and the PDR group, respectively. The difference between two groups was statistically significant (Z=-2.13, P=0.033). The range of the iris vascular fluorescein leakage was 20.00(10.75, 75.00)° and 135.00(60.00, 182.50)° in the severe NPDR group and the PDR group, respectively. There was significant difference between two groups (Z=-4.23, P<0.001). Neovascularization of the iris was not found in all patients with PDR by slit-lamp microscope examination, but was found in 8 eyes by IFA and iris indocyanine green angiography. UWFA findings demonstrated that there was no strong fluorescence in retinal neovascularization elsewhere and/or neovascularization of the optic disc in severe NPDR patients. However, that could be seen in all PDR patients. Among them, 10 eyes had terminally petaloid or focal fluorescein leakage in macular area.
Iris angiography combined with UWFA and quantitative analysis method could not only accurately assess the degree of fundus lesions, but also obtain more accurate and comprehensive iris vascular information, which provided help for comprehensive and personalized treatment of DR. To investigate the effect of anti-vascular endothelial growth factor (VEGF) therapy on the expression levels of transforming growth factor-beta (TGF-β) and its related microRNAs in the vitreous of patients with proliferative diabetic retinopathy (PDR).
This cross-sectional study included 67 patients (67 eyes), 38 males and 29 females, aged (54.37±11.70) years, who underwent vitrectomy from June 2020 to February 2021. There were 45 PDR patients (45 eyes), including 29 patients (29 eyes) without anti-VEGF therapy in the disease group and 16 patients (16 eyes) who were admitted at 7 days after anti-VEGF therapy in the treatment group. The other 22 idiopathic macular hole patients (22 eyes) were in the negative control group. The microRNA (hsa-miR-24-3p and hsa-miR-197-3p) levels in the vitreous of 36 patients (12 cases randomly chosen from each group) were detected by quantitative reverse transcription polymerase chain reaction. The levels of TGF-β and VEGF-A in the vitreous of 67 patients were detected by enzyme-linked immunosorbent assay. Target gene prediction of hsa-miR-24-3p and hsa-miR-197-3p was performed on RNAhybrid, miRanda and TargetScan7.2 databases, and pathway enrichment analyses were conducted for all target mRNAs. One-way ANOVA was used to compare the levels of growth factors and microRNAs among the three groups, and the least significant difference method was used for multiple comparisons between groups. Pearson correlation test was used to analyze the correlation between growth factors and microRNAs.
The expression levels of VEGF-A, TGF-β, hsa-miR-24-3p and hsa-miR-197-3p were (158.15±17.72) pg/ml, (640.47±24.80) pg/ml, 0.81±0.11 and 1.07±0.15 in the control group, (1 047.54±26.61) pg/ml, (3 553.17±92.61) pg/ml, 8.50±2.33 and 12.23±3.38 in the disease group, and (778.10±27.73) pg/ml, (3 376.02±78.83) pg/ml, 4.54±0.67 and 3.90±0.65 in the treatment group, respectively. All indicators were significantly higher in the disease group than those in the control group (F=355.581, 440.538, 7.546 and 7.546; all P<0.05). The expression levels of VEGF-A, hsa-miR-24-3p and hsa-miR-197-3p in the treatment group were significantly lower than those in the disease group (allP<0.05). The concentration of TGF-β was not statistically significantly lower in the treatment group compared to the disease group. The concentrations of VEGF-A and TGF-β were significantly positively correlated with the expression levels of hsa-miR-24-3p and hsa-miR-197-3p in the vitreous of randomly chosen 36 patients (r=0.48, 0.51, 0.40 and 0.42; all P<0.05). Pathway enrichment analysis showed that some target mRNAs of hsa-miR-24-3p and hsa-miR-197-3p were involved in VEGF and TGF-β signal pathways.
In the vitreous of patients with PDR, hsa-miR-24-3p and hsa-miR-197-3p were positively related to VEGF-A and TGF-β, and may be potential risk factors. Anti-VEGF treatment can significantly reduce the expression level of TGF-β-related microRNAs, namely hsa-miR-24-3p and hsa-miR-197-3p, but cannot effectively reduce the concentration of TGF-β, suggesting that combined anti-TGF treatment may be beneficial for delaying the progression of PDR. Furthermore, it may be a new research direction of PDR to validate the target mRNAs of hsa-miR-24-3p and hsa-miR-197-3p involved in VEGF and TGF-β signal pathways. To compare the clinical efficacy of a recombinant bovine basic fibroblast growth factor (rb-bFGF) gel and a gel matrix in the treatment of moderate dry eye.
It was a prospective random double-blind controlled study. One hundred patients diagnosed as moderate dry eye in Eye Institute and Affiliated Xiamen Eye Center of Xiamen University, Union Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology, Beijing Tongren Hospital, Capital Medical University, Eye & ENT Hospital of Fudan University and Zhongshan Ophthalmic Center from August 2015 to April 2019 were divided into two groups: experimental group and control group. Two groups of patients were allocated to receive either a rb-bFGF gel or a gel matrix 4 times per day for 4 weeks. Subjective symptoms, break-up time of the tear film (BUT), Schirmer Ⅰ test (SⅠt) and corneal fluorescein sodium staining were assessed at baseline, 2 and 4 weeks after treatment. Bulbar impression cytology was evaluated at baseline and 4 weeks after treatment. Irritation of the rb-bFGF gel and the gel matrix was estimated after treatment. T test, Wilcoxon signed-rank test or Mann-Whitney U test was used for quantitative data, and Chi-square test was used for enumerative data.
Eighty-four subjects were included for statistical analyses after the exclusion of 16 subjects who were lost for followup, with an age of 43±14 years. There were 42 cases in the experimental group and the control group, respectively. There was no statistically significant difference between the two groups in demographic baseline characteristics before treatment (P>0.05). The total score of subjective symptoms was 7.17±3.60 and 5.95±3.25 at 2 and 4 weeks after therapy in the experimental group, which were lower than 9.48±3.88 before treatment (t=6.226, 6.563; both P<0.05); in the control group, it was 7.01±3.25 and 6.32±3.85 at 2 and 4 weeks after treatment, with a significant reduction in comparison with that before treatment (9.15±3.58;t=4.693, 4.726; both P<0.05). The BUT was 4.41 (2.79, 5.12) s at 2 weeks after therapy in the control group, which was of no statistical difference compared with 3.89 (2.09, 4.25) s before treatment (Z=-1.953, P>0.05). The BUT was 5.21 (3.00, 5.02) s at 4 weeks after therapy in the control group, which was longer than that before treatment (Z=-2.485, P<0.05). The SⅠt score was 7.31 (3.75, 10.00) mm and 8.50 (4.00, 11.00) mm at 2 and 4 weeks after therapy in the experimental group, which were significantly higher than 6.69 (2.00, 8.13) mm before treatment (Z=-2.031, -2.236; both P<0.05); in the control group, it was 6.82 (2.00, 8.25) mm and 6.86 (3.00, 9.25) mm at 2 and 4 weeks after therapy, which were not significantly increased compared with 6.50 (2.00, 7.75) mm before treatment (Z=-0.179, -1.161; both P>0.05). The corneal fluorescein sodium staining points were 5.00 (2.00, 5.00) and 3.71 (0.00, 5.00) at 2 and 4 weeks after therapy in the experimental group, which were significantly lower than 7.10 (5.00, 7.00) before treatment (t=-2.895, -4.639; both P<0.05); those in the control group were 5.52 (0.00, 7.00) and 6.19 (0.75, 6.25) at 2 and 4 weeks after treatment, with a significant reduction in comparison with 8.90 (5.00, 10.50) before treatment (t=-2.776, -1.991; both P<0.05). The differences in the average total score of subjective symptoms, BUT, SIt, and corneal fluorescein sodium staining points between both groups were not statistically significant at each time point. The impression cytology grade was decreased from 1.72 (1.00, 2.00) before treatment to 0.94 (0.00, 2.00) at 4 weeks after therapy in the experimental group (Z=-2.803, P<0.05). The staining grade of conjunctival imprinted cells in the control group was 1.42 (1.00, 2.00) at 4 weeks, which showed no statistical significance compared with 1.56 (1.00, 2.00) before treatment (Z=1.195, P>0.05). The impression cytology grade was significantly reduced in the experimental group compared with the control group at 4 weeks after treatment (Z=-3.308, P<0.05). The number of goblet cells was 10.90 (5.00, 20.00) at 4 weeks after therapy in the experimental group, which was significantly higher than 6.30 (5.00, 8.00) before treatment (Z=-2.383, P<0.05); in the control group, it was 8.36 (4.00, 12.00) at 4 weeks after treatment, with no significant increase in comparison with that before treatment [7.55 (5.00, 11.00)] (Z=-0.095, P>0.05). The number of goblet cells was not significantly increased in the experimental group compared with the control group at 4 weeks after treatment (Z=-1.162, P>0.05). Most patients indicated that the drug was non-irritating, and no patient had intolerable irritation affecting daily lives at 4 weeks after therapy; there was no difference between the two groups (Z=-0.290, P>0.05).
Both the rb-bFGF gel and the gel matrix can effectively improve the symptoms and signs of moderate dry eye. However, compared with the gel matrix, the rb-bFGF gel shows obvious advantages in promoting conjunctival epithelial cell repair and increasing the number of goblet cells. To study the relationship between the clinical characteristics and the histopathological changes in acanthamoeba keratitis.
In this retrospective study, the clinical data and histopathological features of 15 Acanthamoeba keratitis patients (15 eyes) treated in Henan Eye Hospital were collected and analyzed. There were 8 males and 7 females. Mean age was 49.2 years old. The corneal specimens were stained with hematoxylin eosin, periodic acid-schiff, Gomori′s methenamine silver, Gram and Gimsa respectively. The pathological changes and characteristics were observed under microscope, to analyze the relationship between histopathology and clinical features.
All patients had moderate conjunctival congestion. Five patients showed corneal ring stromal infiltration, 8 had central corneal ulcers, 1 had corneal opacity, and 1 had a matrix ulcer. We found corneal neovascularization around the ulcer in 4 eyes and a dense ulcer in 2 eyes. Histopathological examinations with hematoxylin-eosin, periodic acid-Schiff, Gomori′s methenamine silver, Gram and Giemsa stainings revealed round or oval amoebic cysts and amebic trophozoites which were of an elongated oval, spine-or spear-like. Twelve patients (80%) showed corneal stromal purulent inflammation infiltrated by neutrophils. Many degenerated amoebic cysts were found. Degeneration and necrosis of corneal stromal fibers were observed in 3 eyes (20%). There were no neutrophils. Amoebic cysts were seen in necrotic tissue. Ten eyes (66.7%) had no inflammatory cell infiltration areas, with edematous or mildly degenerated corneal fibers, scattered or grouped cysts and trophozoites.
Clinical manifestations of acanthamoeba keratitis are various. Acanthamoeba cysts and trophozoites also exist in the transparent corneal area on the histopathological examination. This is important for the therapeutic outcomes. (Chin J Ophthalmol, 2021, 57: 939-943)
It is a case found during routine reexamination one year after implantable Collamer lens (ICL) implantation. The patient had no complaints. The naked eye visual acuity of the left eye was 1.0, and abnormal blood vessels were seen in the supranasal retina. After fluorescein fundus angiography and sweep source OCTA, it was finally diagnosed as retinoschisis with microvascular anomalies of the left eye. This case suggests that the fundus of patients with high myopia without complaint should also be examined in detail and comprehensively. In addition to paying attention to peripheral retinopathy, the posterior pole and middle peripheral retina should be carefully examined, especially the areas that cannot be covered by conventional OCT.(Chin J Ophthalmol, 2021, 57: 944-945)
A 52-year-old male presented to Beijing Tongren Hospital with a 5-year history of bilateral dry eyes. He had been diagnosed with Steven-Johnson syndrome due to allergy to cold medication. On examination, the Schirmer I test value was 0 mm, the tear break-up time was 0 s, and corneal fluorescein staining showed corneal epithelial erosion with partial fusion in both eyes. The clinical diagnosis was dry eyes (severe) and Steven-Johnson syndrome. No significant effect was observed after use of artificial tears and anti-inflammatory eye drops and tear punctal embolization. Then an insular infraorbital neurovascular pedicle labial salivary gland transplantation was performed in the right eye. During the 1-year postoperative follow-up, the symptoms and signs were significantly improved in the operated eye, and the surgical results were satisfactory. (Chin J Ophthalmol, 2021, 57: 946-948)

Eye examination: corrected visual acuity was 0.8 for the right eye and 0.4 for the left eye; IOP normal; There was no obvious abnormality in the anterior segment of both eyes; Multiple Elschnig spots in the fundus of both eyes (
Pathological myopia, a blinding eye disease, is the most common cause of visual impairment in Asian countries. The most obvious features of pathological myopia are the elongation of the eye axis, the appearance of posterior scleral staphyloma, and even degenerative changes in the retina and choroid, resulting in corresponding complications and ultimately leading to marked visual impairment. Controlling the elongated eye axis is a key factor in preventing the complications of pathological myopia. Posterior scleral reinforcement is the main surgical method to delay the elongation of the eye axis and treat the posterior scleral staphyloma. Although most studies have confirmed that posterior scleral reinforcement is effective in delaying axial elongation and treating myopia, some scholars hold negative views on this surgery. This article summarizes the relevant research results of posterior scleral reinforcement surgery in the treatment of pathological myopia, concerning patients′ vision, refractive power, eye axis, and corneal curvature, and discusses the effectiveness of the surgery.
