2020 marks the 70th anniversary of the founding of Chinese Journal of Ophthalmology. Thanks to the efforts of previous editors-in-chief, editorial board and editorial department, Chinese Journal of Ophthalmology has not forgotten its original intention, kept pace with the times, and guided and witnessed the history of rapid development of modern ophthalmology in China. In this volume, this journal will publish retrospective articles on the development process and achievements in related fields of ophthalmology one after another, in order to review the past and learn new things, and promote the prosperity and development of ophthalmology in China.
Corneal refractive surgery in China, represented by laser corneal surgery, has undergone great changes in recent 30 years. Almost one million corneal refractive surgeries are performed each year. With the rapid technological innovation, the corrective effect is constantly higher, and complications are continuously controlled and reduced. The team of practitioners keeps expanding, and the academic strength is approaching the international level. With the application and promotion of various new technologies, the formulation of relevant norms and the increase of academic researches, corneal refractive surgery in China has stepped into the stage of diversification, intelligence and standardization. However, we should recognize the shortcomings while facing the great achievements. More efforts are still needed to provide the best visual quality and natural vision correction effect for patients. We would like to send congratulations on the 70th anniversary of Chinese Journal of Ophthalmology with this article. (Chin J Ophthalmol, 2020, 56: 81-85)
Small incision lenticule extraction (SMILE) has been widely used in the treatment of myopia and astigmatism due to its small incision (2 mm), without open corneal flap, rapid recovery of the ocular surface function and good comfort. However, it is the microincision and the potential cavity under the corneal cap that provide a suitable environment for microbial nourishment. In this article, the characteristics of SMILE and the particularity of postoperative infection are stated, and the key points of risk management and control during the period of operation as well as the principles of identification and treatment of infection after SMILE surgery are put forward, so as to enhance the perioperative preparation in SMILE and make sure of the safety and high quality for myopic eyes. (Chin J Ophthalmol, 2020, 56: 86-88)
Myopic refractive surgery has developed vigorously in China during the past 30 years, and the number of surgeries is increasing year by year. Since intraocular refractive surgery has a wide range of correction, no need to cut the corneal tissue and a stable visual quality, it has been gradually promoted in clinical practice. Specialists from the Optometry Group of Chinese Ophthalmological Society have given guidance on the ocular indications, age, diopters and other aspects for the posterior chamber intraocular lens implantation. In this article, the authors expound that intraocular refractive surgery should not be recommended for low and moderate myopic eyes, neither for some special occupations at present. The complications of intraocular refractive surgery and the countermeasures are also discussed. The authors also put forward the principles of customized myopic refractive surgery and emphasize that the clinicians should be more cautious about the diopter range selection, occupational need evaluation, individual differences and complications of intraocular refractive surgery. To investigate the application value of Pentacam combined with Corvis ST in evaluation of the changes of corneal biomechanics after femtosecond laser small incision lenticule extraction (SMILE) in Chinese myopia with an irregular cornea.
The clinical records for 104 eyes of 57 patients who received SMILE in the Refractive Center of Beijing Tongren Hospital during January 2018 and May 2018 were collected. According to the keratoconus severity index (KSI), they were divided into two groups: regular corneal group (KSI<15%) and irregular corneal group (KSI: 15% to 25%). In both groups, the anterior corneal surface radius curvature was>7.25 mm (K<46.50 diopters), the posterior corneal surface radius curvature was>5.90 mm, the thinnest pachymetry was>490 μm, and best corrected visual acuity was ≥1.0. The vision, refraction, and corneal biomechanics before and after SMILE were assessed. The Topographic and Biomechanics Index (TBI) was analyzed by Pentacam combined with Corvis ST.
Before SMILE, the Corvis Biomechanical Index (CBI), TBI, and Belin/Ambrósio Deviation Normalized Index (BADD) of the irregular corneal group were significantly higher (t=-2.17, -6.78, -4.37, P<0.05) than the regular corneal group, while the stiffness parameter (SPA1) was significantly lower (t=2.58, P=0.011) compared to the regular corneal group (P<0.05). In the irregular group, the TBI was (0.28±0.2); the maximum value was 0.03, and the minimum value was 0.43. The CBI was (0.09±0.21); the maximum value was 0.00, and the minimum value was 0.54. The BADD was (1.33±0.47); the maximum value was 0.42, and the minimum value was 2.26. In the regular group, the TBI was (0.05±0.08); the maximum value was 0.00, and the minimum value was 0.20. The CBI was (0.01±0.03); the maximum value was 0.00, and the minimum value was 0.17. The BADD was (0.92±0.46); the maximum value was 0.00, and the minimum value was 1.64. There was no significant difference between two groups in age (t=0.20, P=0.508), central corneal thickness (t=1.64, P=0.104), biomechanical corrected IOP (t=0.73, P=0.468), max inverse radius (t=-0.24, P=0.815), spherical equivalent (t=-0.97, P=0.335), and best corrected visual acuity (t=0.21, P=0.833). After SMILE, the deformation amplitude in the irregular group was significantly higher at 1 month and 3 months (t=-3.13, -3.09, P<0.05). The irregular group had a significantly higher deformation amplitude ratio at 1 week, 1 month, and 1 year (t=-2.72, -3.39, -2.51, P<0.05). The SPA1 in the irregular group was significantly lower than the regular group at 1 week, 1 month, and 3 months (t=2.11, 2.73, 3.70, P=0.335, 0.010,<0.001). The changes of deformation amplitude (t=0.50, -1.10, -0.73, 2.12, P>0.05), max inverse radius (t=-1.52, -1.41, 0.01, -0.79, P>0.05), and SPA1(t=0.89, 0.90, 1.12, 0.90, P>0.05) after SMILE were similar between the irregular and regular groups, except that at 1 month after SIMILE, the deformation amplitude ratio changed more significantly in the irregular group (t=-3.01, P=0.003).
The changes of corneal biomechanics in the groups of regular cornea and irregular cornea were stable with no significant difference during 1 year of post-SMILE. The diagnosis based on the corneal topography and corneal biomechanics is of certain significance for the screening of early keratoconus before keratorefractive surgery. To investigate the influence factors and differences of abnormal posterior corneal elevation by Pentacam system and Corvis ST.
This retrospective case series study included 227 eyes of 144 patients (90 males, 139 eyes; 54 females, 88 eyes) from December 2017 to October 2018 who were going to receive corneal refractive surgery at the Corneal Refraction Department of Qingdao Eye Hospital. The general data of the patients including gender, age, refractive parameters, optimal correction of spherical and cylindrical diopters were collected. All patients underwent Pentacam system and Corvis ST measurement. According to the back difference (BD) of Pentacam parameters, BD<12 μm was set as the control group (59 patients, 118 eyes) and BD≥12 μm as the high BD group (85 patients, 109 eyes). In the high BD group, BD≤16 μm was set as the suspicious group (44 patients, 53 eyes), while BD>16 μm was set as the abnormal group (41 patients, 56 eyes). Seven parameters of Pentacam and 15 parameters of Corvis ST were selected. The Pentacam parameters included BD, anterior surface keratometry (ASK), posterior surface keratometry (PSK), anterior surface astigmatism (AAstig), posterior surface astigmatism (PAstig), central corneal thickness (CCT), and corneal diameter (W-W). The parameters of Corvis ST included the first applanation time (AT1), the first applanation length (AL1), the first applanation velocity (AV1), the second applanation time (AT2), the second applanation length (AL2), the second applanation velocity (AV2), highest concavity time (HCT), highest concavity peak distance (HC-PD), highest concavity deformation amplitude (HC-DA), highest concavity radius (HC-R), the ratio of deformation amplitude (DA ratio), Integr. Radius, corneal thickness thinnest/pachymetric progression (ARTh), SPA1 (resultant pressure divided by deflection amplitude at the first applanation), and the Corvis Biomechanical Index (CBI). The comparison between the groups was analyzed with Independent sample t test, Kruskal-Wallis H test, and Bonferroni test. Spearman rank correlation analysis was used to explore the correlation factors of BD, and the main factors affecting BD were found through multiple linear regression.
There were no statistically significant differences between the control group and the high BD group in age, spherical diopters, and cylindrical diopters (t=-3.311, -1.808, -2.359; P=0.071, 0.072, 0.121, respectively). In Pentacam parameters, ASK, PSK, PAstig, and W-W showed significant differences among groups (Z=18.492, 31.547, 10.773, 70.167; P<0.05). AAstig and CCT showed no statistical difference between groups (P>0.05). Compared with the control group [42.80 (41.98, 44.00)], ASK increased in the abnormal group [43.40 (42.20, 44.40)] significantly (t=-4.292; P<0.05). PSK of the suspicious group [-6.50 (-6.60, -6.35)] and the abnormal group [-6.50 (-6.70, -6.33)] increased significantly compared with the control group [-6.30 (-6.50, -6.20)] (t=4.492, 4.618; P<0.05). Compared with the control group [0.40 (0.30, 0.50)], PAstig of the suspicious group [0.40 (0.30, 0.40)] and the abnormal group [0.40 (0.30, 0.40)] increased significantly (t=2.796, 2.515; P=0.016, 0.036). Compared with the control group [11.50 (11.40, 11.80)], W-W of the suspicious group [11.40 (11.00, 11.60)] and the abnormal group [11.10 (10.90, 11.30)] decreased, and W-W of the abnormal group also decreased significantly compared with the suspicious group (t=3.235, 8.353, 4.282; P<0.05). The correlation analysis between BD and Pentacam parameters of patients in each group showed that BD was negatively correlated with W-W (r=-0.614, -0.304, -0.396, -0.661, P<0.05) in the control group, the suspicious group, the abnormal group, and all patients, while BD had a low correlation with other parameters or no significant correlation. The correlation analysis of BD and Corvis ST parameters in patients showed that only in the suspicious group, BD was positively correlated with AV1, HCT, and HC-DA (r=0.332, 0.361, 0.382, P<0.05), while no significant correlation was found between BD and other Corvis ST parameters in each group. In order to further explore the main factors affecting BD, Pentacam parameters and Corvis ST parameters were selected as independent variables with BD as the dependent variable to establish a multivariate linear regression analysis model. There was no collinearity between variables W-W, ASK, PSK, HC-PD, SPA1, and CCT (tolerance<0.100). The equation test result wasF=37.221, P<0.001, adjusted r2=0.504, and the fitting was good.
Among the Pentacam parameters, W-W, ASK, and PSK are the main factors affecting the change of BD. HC-PD and SPA1 in the Corvis ST parameters may also have some influence on BD. The Pentacam system combined with Corvis ST is a very useful differential diagnosis system for patients with abnormal BD. (Chin J Ophthalmol, 2020, 56:110-117)
To investigate mesopic contrast sensitivity (CS) and high-order aberrations after femtosecond laser-assisted LASIK (FS-LASIK), wavefront-guided femtosecond LASIK (WF-LASIK) and small incision lenticule extraction (SMILE) for myopia and astigmatism.
In this case-control study, 212 eyes of 108 patients with myopia and astigmatism from September 2014 to September 2016 were treated by refractive surgery. There were 34 males (66 eyes) and 74 females (146 = eyes), aged 27 (24, 32) years. They were divided into 3 groups, with 37 patients (71 eyes), 35 patients (69 eyes) and 36 patients (72 eyes) treated by FS-LASIK, WF-LASIK and SMILE, respectively. Refraction diopters, corneal tomography, wavefront aberrations and contrast sensitivity were evaluated before and at 12 months after surgery. The results of different groups were compared by rank sum test.

Fifty-nine eyes (83.10%) treated by FS-LASIK, 60 eyes (86.96%) by WF-LASIK and 61 eyes (84.72%) by SMILE had better postoperative uncorrected distance visual acuity than preoperative corrected distance visual acuity. Surface regularity index of the WF-LASIK group decreased from 0.11 (0.07, 0.28) to 0.07 (0.06, 0.20) (Z=-2.662, P=0.008), which was less than that of the FS-LASIK and SMILE groups (Z=-3.236, -3.118, P=0.001, 0.000). Surface asymmetry index of the WF-LASIK group increased from 0.34 (0.25, 0.43) to 0.38 (0.30, 0.52) (Z=-2.140, P=0.032), which was less than that of the FS-LASIK and SMILE groups (Z=-2.910, -3.779, P=0.004, 0.000). The root mean square of high-order of the FS-LASIK group increased from 0.32 (0.27, 0.41) to 0.53 (0.40, 0.65) (Z=-6.228, P=0.000), which was more than that of the WF-LASIK and SMILE groups (Z=-3.595, -4.806, P=0.000, 0.000). Coma of the WF-LASIK group increased from 0.16 (0.11, 0.24) to 0.26 (0.15, 0.37) (Z=-3.816, P=0.000), which was less than that of the FS-LASIK group (Z=-3.528, P=0.000). Spherical aberration of the FS-LASIK and WF-LASIK groups increased from 0.10 (0.05, 0.21) and 0.08 (0.04, 0.12) to 0.24 (0.13, 0.34) and 0.15 (0.09, 0.26), respectively (Z=-5.141, -4.864, P=0.000, 0.000), which were more than that of the SMILE group (Z=-2.549, -4.682, P=0.011, 0.000). CS of the WF-LASIK group at 12 cycles per degree (c/d) increased from 1.34 (1.18, 1.48) to 1.48 (1.34, 1.63) (Z=-2.985, P=0.003), which was more than that of the FS-LASIK and SMILE groups (Z=-3.194, -2.977, P=0.001, 0.003). CS with glare of the WF-LASIK group at 12.0 c/d increased from 1.26 (1.18, 1.34) to 1.34 (1.34, 1.63) (Z=-3.608, P=0.000), which was more than that of the FS-LASIK and SMILE groups (Z=-4.033, -2.913, P=0.000, 0.004).
FS-LASIK, WF-LASIK and SMILE achieved ideal visual outcomes at 12 months postoperatively. WF-LASIK treated eyes showed the best corneal symmetry, the least increase of high-order aberrations and the best visual quality among three kinds of surgeries. (Chin J Ophthalmol, 2020, 56: 118-125)
To discuss the efficacy and security of double-incision extracapsular cataract extraction (ECCE) in the treatment of hard-nucleus cataract with low corneal endothelial cell density.
This was a retrospective case series study. Forty-six patients (47 eyes) with hard-nucleus cataract and low corneal endothelial cell density were enrolled at Shandong Eye Hospital from June 2009 to December 2018, including 22 males and 24 females, aged 50 to 74 (63.8±6.3) years. Preoperative corneal endothelial cell density was less than 1 000 cells/mm2, and the cataract nuclear hardness was equal to or greater than grade IV. According to the surgical methods, the patients were divided into the single-incision ECCE group (24 eyes) and the double-incision ECCE group (23 eyes). The surgical procedures for the double-incision group were as follows. First, a superior scleral pre-incision was made. Then the conventional capsulorhexis was conducted through a 2.6 mm transparent corneal tunnel incision at the temporal or the nasal side, after which the hydro-dissection was performed. Next, the surgeon cut the pre-incision at the sclera, delivered the lens nucleus, sutured the scleral incision and removed the residual cortical materials from the corneal incision. Finally, a foldable intraocular lens was implanted, and the viscoelastic substance was removed. The intraoperative anterior chamber condition and the postoperative corneal edema condition were monitored. During the 6 month follow-up after surgeries, the endothelium cell density, visual acuity and astigmatism in the two groups were compared. The χ 2 test was used to compare the counting data, and the t test was used to compare the measurement data.
There was no statistically significant difference (t=1.12, P=0.28) in the preoperative corneal endothelial cell density between the double-incision and single-incision ECCE groups, which was (827±164) cells/mm2 and (802±121) cells/mm2, respectively. At 6 months after operation, in the double-incision and single-incision groups, the endothelial cell density was (793±147) cells/mm2 and (706±101) cells/mm2, respectively, and the difference was statistically significant (t=4.37, P<0.01). The percentage of corneal endothelial cell loss was 4.16%±3.12% and 11.69%±2.96%, respectively, and the difference was also statistically significant (t=9.52, P<0.01). The hexagon loss rate of corneal endothelial cells was 9.67%±6.11% and 28.33%±8.39%, respectively, and the difference was statistically significant (t=5.52, P<0.05). In the follow-up of 6 months, none of the eyes in the double-incision ECCE group suffered corneal endothelial decompensation compared with 3 eyes in the single-incision ECCE group. There were no statistically significant differences in postoperative astigmatism and surgically induced astigmatism between the two groups (t=-0.71, 0.15; P>0.05).
The double-incision ECCE, in which the lens nucleus is delivered through a scleral incision and other procedures are conducted through a corneal tunnel incision, is safe and effective for cataract patients with hard nucleus and low corneal endothelial cell density. According to the administration of deionized water or 0.5, 1.0, 1.5 mg/ml berberine solution, they were divided into the general control group and the general A, B, and C group. Dosing with both eyes, each eye was given a single dose, and then it was given multiple times for 4 weeks after observation for 72h. After the corneal epithelium injury model made in the right eye of rabbits in the corneal injury groups, they were divided into a corneal injury control group and a corneal injury group A, B, and C according to the administration of deionized water or 0.5, 1.0, 1.5 mg/ml berberine solution. there were 5 rabbits (15 eyes) in each group, and the solutions were given continuously for 1 week. The rabbits in the general group were observed their behavioral changes, ocular surface and iris were scored by Draize eye irritation test scoring system. IOP was measured at different time points. Electroretinogram (ERG) was used to detect b-wave amplitude. In the corneal injury group, corneal epithelial defect repairment was observed at 1, 2, 3, 4, 5, 6, and 7 days after the corneal defect. Corneal histopathology observation after discontinuation of all rabbits. The pH value of rabbit tears was described by the paired t test, and the score of Draize eye irritation test were described by the rank-sum test. The analysis of variance and SNK-q were used for IOP, electroretinogram b-wave amplitude, corneal epithelial injury area and repair time.
No abnormal behavior was observed in the general group rabbits after single and multiple administration. There was no significant difference in the Draize eye irritation score among the general control group and the general group A, B, C at 1, 2, and 4 weeks of multiple administrations. Among them, the Draize eye irritation score of the general group C was 7 (0, 12), 6 (0, 10), 6 (0, 16) points (χ2=1.640, 0.265, 1.963, 1.381; P>0.05).There were no significant difference in IOP at different times among the general control group and the general group A, B, C at different times (F=0.065, 0.292, 0.015, 0.041; P>0.05). Before multiple administrations and after administration at 2, 4 weeks, the b-wave amplitudes of the general control group were (127.75±17.12), (129.18±15.83), (128.81±13.58) μV, and the general group A were (130.68).±18.75), (131.38±16.96), (130.62±12.18) μV,and the general group B were (128.00±16.74), (128.44±16.64), (129.06±16.16) μV, and the general group C were (131.81±19.37), (132.13±18.36), (129.94±12.60) μV. There was no statistically significant difference in b-wave amplitude in the groups at different times before and after administration ( F=0.037, 0.011, 0.017, 0.702; P>0.05). There was no significant difference in the results of corneal histopathology among the general control group and the general group A, B, C. The area of corneal epithelial defect in each corneal injury group was statistically significant at different time (F=5.316, 25.864, 127.613; P<0.05). The corneal injury control group compared with the corneal injury group A, B, C, the corneal epithelial defect area in the corneal injury group C was significantly larger than the other three groups, with statistical differences (q=5.153, 10.313, 6.976; P<0.05). The repair time of corneal epithelial in control group and the group A,B,C of corneal injury were (83.0±1.85), (82.9±2.07), (83.7±2.09) and (101.6±2.20) h. The corneal epithelium defect repair time in group C was longer and the difference was statistically significant (F=301.437, P=0.000). Comparing the corneal injury control group and corneal injury group A and B, there was no statistical difference in the repair time of corneal epithelial defect (F=0.813, P=0.450). After repair, there was no significant difference in the pathological results of the corneal tissue between the corneal injury groups.
Berberine solution in rabbit eyes with topical application was safety, and has no obvious toxic effect on the ocular surface and ERG of normal rabbits. 1.5 mg/ml berberine solution delayed the repair of experimental corneal epithelial defect, but had no effect on the integrity of corneal tissue after repair. To investigate the corneal epithelial thickness changes between small incision lenticule extraction (SMILE) and femtosecond laser-assisted LASIK (FS-LASIK), study the related factors and analyze the relationship between the changes in corneal epithelial thickness and corneal aberration.
This case control study included 59 patients (118 eyes), 27 males and 32 females, aged 25.64±5.57 years, who were scheduled for SMILE or FS-LASIK for treatment of myopia with or without myopic astigmatism at the First Affiliated Hospital of Soochow University between October 2016 and May 2017. All patients were divided into two groups according to the surgery. Epithelial thickness was obtained in nine zones with Fourier-domain optical coherence tomography across a 5-mm diameter centered by the pupil before surgery and at 1 week, 1 month, 3 months and 6 months postoperatively. The observed changes were analyzed by linear-regression analysis with the central corneal thickness decrement, ablation rate and corneal aberration. Comparison between two groups was analyzed by independent sample t-test, analysis of variance and Mann-Whitney U test.
There was no significant difference in corneal epithelial thickness of nine zones between SMILE and FS-LASIK for low to moderate myopia (manifest refraction sphere equivalent<-6.00 diopters) and high myopia (manifest refraction sphere equivalent ≥-6.00 to -10.00 diopters), respectively, before surgery (P>0.05). At 1 month, no significant difference existed in corneal epithelial thickness increment in nine zones between FS-LASIK and SMILE for low to moderate myopia (P>0.05), but a larger increase was observed in the superior (t=2.670, P=0.010), superonasal (t=2.506, P=0.015) and nasal (t=2.831, P=0.007) zones following FS-LASIK [(4.2±3.2), (3.4±2.7) and (3.7±2.5) μm] than SMILE [(2.2±2.2), (1.6±2.5) and (1.7±2.6) μm] for high myopia. At 3 months, a larger increase in corneal epithelial thickness was observed in the superior ( t=2.703, P=0.009) zone following FS-LASIK than SMILE for low to moderate myopia, and the same increase was observed in the central (t=2.660, P=0.010), superior (t=3.229, P=0.002), nasal (t=2.420, P=0.019), inferonasal (t=2.651, P=0.011), inferotemporal (t=2.153, P=0.036) and superotemporal (t=2.281, P=0.027) zones after FS-LASIK [(6.7±2.6), (5.0±2.1), (3.9±1.9), (4.8±1.8), (7.5±2.5) and (6.4±2.7) μm] than SMILE [(4.7±2.9), (2.9±2.5), (2.4±2.4), (3.3±2.2), (6.0±2.4) and (4.8±2.4) μm] for high myopia. At 6 months, a larger increase in corneal epithelial thickness was observed in the superior ( t=3.340, P=0.001) and nasal (t=1.952, P=0.055) zones following FS-LASIK than SMILE for low to moderate myopia, and the same increase was observed in the superior (t=2.332, P=0.024) and inferonasal (t=2.172, P=0.034) zones after FS-LASIK than SMILEfor high myopia. The central corneal thickness decrement after SMILE was much more than FS-LASIK both in the low to moderate myopia and high myopia groups (P<0.05). The average corneal epithelial thickness increment correlated positively with the central corneal thickness decrement and ablation rate at 1 month, 3 months and 6 months postoperatively (P<0.01). For SMILE, the total higher orders aberration (0.81±0.26, 0.79±0.28 and 0.81±0.31) and spherical aberration (0.50±0.21, 0.48±0.20 and 0.52±0.23) were less than FS-LASIK (0.97±0.34, 0.97±0.33 and 0.93±0.32; 0.72±0.25, 0.66±0.30 and 0.71±0.25) at 1 month, 3 months and 6 months postoperatively (P<0.05). Furthermore, the corneal aberration increment correlated positively with the average corneal epithelial thickness increment (P<0.05).
The corneal epithelial thickness increment after SMILE was less than FS-LASIK. SMILE had better uniformity of the corneal epithelial thickness increment at the observed zones, which may explain the finding that the postoperative spherical aberration of SMILE was less than FS-LASIK. The patient was a female with red eyes and blurred vision after scratching of the wheat grain shell in her left eye, and the anti-inflammatory treatment effect was not good. Pigmented protuberance in the central cornea of the left eye, epithelial defects at the upper temporal and inferior temporal infiltration foci, and multiple satellite foci scattered on the lower corneal surface, with feathered borders. The results of fungal culture and in vivo laser confocal microscopy indicated that the fungus of the genus Acanthopanax was diagnosed as Acanthopanax fungal keratitis by sequencing. After drug susceptibility test, the antifungal treatment was not effective, and the left eye was removed.(Chinese Journal of Ophthalmology, 2020, 56:138-140)
Surgeon IntroductionGao Hua, Ph.D., chief physician, professor, doctoral supervisor. Deputy Director of Shandong Institute of Ophthalmology and Vice President of Shandong Eye Hospital. Member of Keratology Group of Ophthalmology Branch of Chinese Medical Association, Secretary-General and Standing Committee Member of Ophthalmology Branch of Chinese Ethnic Health Association, Taishan Scholar Young Expert, and International Member of American Academy of Ophthalmology. Clinically, he is mainly engaged in the diagnosis and treatment of corneal and ocular surface diseases, and completes more than 1,000 cases of corneal surgery every year.
The patient was a 49-year-old male. He was seen in the Department of Ophthalmology of Peking Union Medical College Hospital because of painless visual loss in his left eye for 2 years. Deny history of night blindness and similar family history. There were no obvious abnormalities in the whole body examination. Eye examination: the visual acuity of the right eye was 0.8, and the left eye was manual, both of which could not be corrected; IOP normal; Mild cortical cataract in both eyes, no other obvious abnormalities in the anterior segment; For both fundus, see
With the extensive development of small incision lenticule extraction (SMILE), a large number of human corneal stromal lenticules were extracted integrally during the operation, which led some experts to study the reuse of the lenticules. In experimental research, the lenticules were used to culture fibroblasts, construct corneal scaffolds and describe the biomechanical behaviors of cornea by cytobiology, immunology and biomechanics. In clinical study, the lenticules had been successfully reimplanted into autologous or allogenic cornea of human subjects for correcting hyperopia and presbyopic, patching corneal perforation and treating defective keratopathy and so on. The outer retinal tubular structure (ORT) indicates a round or oval-shaped structure with a highly reflective border and a relatively low reflective lumen on spectrum-domain coherence tomography. ORTs are located in the outer nuclear layer accompanied by disruption and curling of the external limiting membrane and retinal pigment epithelial atrophy. Histopathological researches have revealed that ORTs are some kind of remodeling of the outer retina especially photoreceptors under pathological conditions, representing an advanced damage to the outer retina and a common final pathway of various retinal degenerative diseases. ORTs are common in neovascular age-related macular degeneration. They show some similarities in morphology with inter retinal fluid or cystoid edema on spectrum-domain coherence tomography, but they are not an indication for neovascular activity and retreatment. Their response to anti-vascular endothelial growth factor is poor, and the visual prognosis is not optimistic. Therefore, raising awareness of ORTs is very important for guiding clinical treatment and judging prognosis. (Chin J Ophthalmol, 2020, 56: 149-154)
Pterygium is a common ocular surface disease in China, which is mainly treated by surgery. Among them, pterygium resection combined with various types of tissue material transplantation has the best effect, and the recurrence rate is significantly lower than that of pterygium resection alone. In order to further simplify the surgical operation and improve the postoperative discomfort symptoms, the fixation mode of the graft changed from traditional suture suture to various fixation methods without suture, all of which achieved good results. This paper summarizes and analyzes the types of graft and their fixation methods commonly used in pterygium resection, in order to provide reference for the clinical work of ophthalmology.(Chinese Journal of Ophthalmology, 2020, 56:155-160)
