When the first sunshine of 2015 shines into the title page of Chinese Journal of Ophthalmology, all our journal runners have started to embark on the journey of the new year with vigorous steps, to look forward to and imagine the dreams that spring will bring us, and to face and meet the challenges of external competition to the journal.
The eye care problems related to optometry involve a wide range, including visual problems during eye disease recovery, visual quality in surgical or non-surgical refractive corrections, and the etiological investigation of functional eye diseases like myopia. This article covers the current challenges to visual health care and the academic developments and contributions of optometry in China, including fundamental researches of myopia, refractive surgery and visual quality, and functional eye disease investigations. Some of the researches have certain impacts both domestically and overseas. Furthermore, scientific evidences to solve clinical problems and the current academic focuses that we should pay attention to are provided.(Chin J Ophthalmol, 2015, 51: 3-7)
The prevalence of high myopia in teenagers is rising. Due to both genetic factors and environmental factors, most of myopia occurs in early age with rapid progress, and the prevention and control are very difficult. Orthokeratology is considered as one of the most effective ways of controlling myopia for children. It is proved effective through many years of clinical studies not only for low to moderate myopia, but also for high myopia with astigmatism. However, professional knowledge is lacking domestically. This paper introduces the local and overseas research results in recent years, and discusses the necessity and feasibility of high diopter orthokeratology correction, implementation methods and requirements, and safety and effectiveness evaluation standards for reference.(Chin J Ophthalmol, 2015, 51: 8-10)

To compare the accommodation response (AR) to monochromatic targets in subjects with different color vision statuses, and to investigate the role of color vision in the control of accommodation and emmetropization.
It was a case-control study. Accommodation was measured with a dynamic infrared optometer while subjects [17 protans, 47 deutans, and 23 normals; mean age: (20.0±4.4) years] viewed a (1) red on black or (2) green on black vertical square-wave gratings of iso-luminance (3 cycles/deg; 0.9 contrast) in a Badal optic system. The grating stepped 1.00 D towards the eye from an initial position of 0 D until 5.00 D.
With red-black targets, the AR in the protans (AR=1.98 D) was worse than that in the normals (AR=2.55 D) when the accommodation stimulus (AS) was 4.00 D (LSD, P=0.031). The AR in the deutans were worse than that in the normals when the AS was 3.00, 4.00, and 5.00 D (3.00 D: 1.23 D vs. 1.69 D, P=0.002; 4.00 D: 1.89 D vs. 2.55 D, P=0.002; 5.00 D: 2.40 D vs. 3.17 D, P=0.003). With green-black targets, the AR in the protans were worse than that in the normals when the AS was 3.00 and 4.00 D (3.00 D: 1.13 D vs. 1.61 D, P=0.004; 4.00 D: 1.80 D vs. 2.34 D, P=0.021). In the deutans, the AR was worse with stimuli of 3.00, 4.00, and 5.00 D (3.00 D: 1.21 D vs. 1.61 D, P=0.003; 4.00 D: 1.65 D vs. 2.34 D, P<0.001; 5.00 D: 2.36 D vs. 2.93 D,P=0.007). No significant differences between the protans and deutans were found for all the stimulus conditions. In the protans, accommodation to red-black targets was better than that to green-black targets when the stimulus was 2.00, 3.00, and 5.00 D (2.00 D: t=-2.81, P=0.013; 3.00 D: t=-4.55, P<0.001; 5.00 D:t=-3.15, P=0.006). In the deutans, accommodation to red-black targets was better than that to green-black targets when the stimulus was 4.00 D (t=-2.19, P=0.034). In the normals, accommodation to red-black targets were better than that to green-black targets when the stimulus was 2.00, 4.00, and 5.00 D (2.00 D: t=-2.57, P=0.017; 4.00 D, t=-2.67, P=0.014; 5.00 D: t=-2.15, P=0.043).
Individuals with a color vision deficiency tend to have a larger accommodative lag than normals. Red targets tend to induce better accommodation response than green ones. Color vision may play a role in the control of accommodation and emmetropization.(Chin J Ophthalmol, 2015, 51: 15-19)
To compare the agreement of the ocular modulation transfer function (MTF) measured by double-pass system and ray tracing wavefront aberrometry, and to analyze the correlations of two MTFs with the visual acuity and contrast sensitivity function results.
Comparative study. Subjects with no ocular diseases were consecutively enrolled in an epidemic study field located at the Dongyangzhuang Health Center, Yongnian County, Handan City, Hebei Province, China. After comprehensive ophthalmic examinations, the mean values of subtracted lower order aberration MTF at 5, 10, 15, 20, 25, and 30 cycle/degree (c/d) spatial frequencies were obtained with a double-pass system (optical quality analysis system Ⅱ, OQASⅡ system) and a ray tracing wavefront aberrometer (iTrace visual function analyzer, iTrace system) in the 4.0 mm and 6.0 mm pupil after dilation, respectively. Paired-sample t test and Bland-Altman analysis were used to compare the difference and agreement of MTFs obtained with two instruments. Correlation analysis was preformed between two MTF measurement results and subjective visual quality including visual acuity and contrast sensitivity function.
Two hundred and fifty-one healthy eyes of 163 subjects were enrolled, aged 30 to 60, mean (44.1±9.7) years, including 139 eyes of 81 males and 112 eyes of 82 females. The mean value of MTF at 5, 10, 15, 20.25, 30 c/d obtained by iTrace in 4.0 mm pupil were 0.730±0.138、0.431±0.159、0.262±0.120、0.169±0.078、0.118±0.053、0.094±0.043. The value obtained by OQASⅡ were 0.347±0.123、0.162±0.086、0.072±0.049、0.042±0.033、0.026±0.022、0.017±0.022, The result of iTrace were all significant higher than OQAS in both 4mm (t=38.72, 28.03, 27.32, 27.59, 29.23, 28.96, P<0.01) and 6.0 mm (t=4.60, 3.19, 9.34, 13.41, 16.96, 20.24, P<0.01) pupil diameter. The iTrace-OQASⅡ MTF difference was smaller in the 6.0 mm pupil. Bland-Altman analysis indicated that the agreement of two instruments was poor, while the limits of agreement improved in the 6.0 mm pupil. The iTrace-derived MTFs were negatively correlated with the subjects′ LogMAR best corrected visual acuity (r=-0.139--0.165, P<0.01) and positively with the log value of contrast sensitivity function at 6 c/d (P<0.05). OQAS-derived MTFs were negatively correlated with LogMAR best corrected visual acuity (r=0.239-0.303, P<0.01) and positively with contrast sensitivity at all spatial frequencies (r=0.156-0.398, P<0.01).
There exists a significant difference and poor agreement in MTF measurement based on the ray tracing wavefront aberrometer and the double-pass system, while the difference becomes smaller in a large pupil diameter. The double-pass system provides a useful tool in clinical optical quality evaluation.(Chin J Ophthalmol, 2015, 51: 20-25)
To investigate the effect of corneal irregularity on on-eye rigid gas permeable (RGP) contact lenses and the corneal shape changed by RGP contact lenses.
Corneal topography was taken for 42 bare eyes of 22 subjects before and 1 month after wearing of the RGP contact lens. Off-eye and on-eye wavefront aberrations of the anterior surface of the RGP contact lens were also measured by corneal topography for the same 42 eyes. Zernike aberrations of the RGP contact lens were examined and compared to those of baseline anterior corneal surface.
For bare eyes, C4, C5, C7 and C8 were significantly different from the baseline 30 minutes after the RGP contact lens was removed. For the on-eye RGP contact lens, three mean Zernike aberrations were significantly different from those off-eye. These were defocus C4 [OD: (1.003±0.131) μm off-eye and (1.065±0.160) μm on-eye, P=0.015; OS: (1.003±0.130) μm off-eye and 1.069 (0.851, 1.594) μm on-eye, P=0.017], main axis astigmatism C5 [OD: (0.072±0.083) μm off-eye and (-0.312±0.232) μm on-eye, P<0.001; OS: (0.066±0.056) μm off-eye and (-0.349±0.242) μm on-eye,P<0.001], and spherical aberration C12 [OD: (0.264±0.039) μm off-eye and (0.295±0.048) μm on-eye,P<0.001; OS: (0.266±0.035) μm off-eye and 0.290 (0.215, 0.471) μm on-eye,P<0.001]. All these three Zernike aberrations of on-eye RGP contact lenses were significantly correlated to those of the baseline corneas (OD:rc4=0.557, rc5=0.596, rc12=0.581, P<0.01; OS:rc4=0.684, rc5=0.497, rc12=0.543, P<0.01).
The results suggest that the surface shape of on-eye RGP contact lenses is not rigid, and its change depends on irregularity of the anterior corneal surface, even if the cornea itself is changed by the RGP contact lens. It is better not to fit for RGP so flatter than the average curvature of the cornea.(Chin J Ophthalmol, 2015, 51: 26-31)
To determine the changes of objective optical quality after orthokeratology.
Prospective, self-controlled study. Optical quality, including objective scatter index (OSI), modulation transfer function (MTF) cutoff frequency, contrast visual acuity (CVA) and objective tear film quality, was assessed with the OQASⅡ instrument before and 1 week, 1 month, and 3 months after orthokeratology. Parameters of corneal astigmatism and e value from topography were also assessed. The data was analyzed using one-way ANOVA.
Cylinder refraction was (-0.39±0.39)D, (-0.65±0.57)D, (-0.79±0.56)D, and (-0.81±0.53)D, respectively, at the four timepoints, and there was a statistically significant difference between before and after therapy (P<0.05). Uncorrected visual acuity was 3.97±0.51, 4.95±0.12, 5.00±0.06, and 5.00±0.05, respectively. The e value was 0.37±0.24, -0.23±0.42, -0.29±0.39, and -0.30±0.37, respectively. The negative values after therapy were significantly different from the value before therapy (P<0.01). The corneal astigmatism in 3 mm diameter was (1.27±0.75) D, (1.06±0.69) D, (1.11±0.68) D , and (1.15±0.67)D, respectively. There was no significant difference in astigmatism between before and after therapy (P≥0.05). After therapy, the OSI increased from 0.59±0.47 before therapy to 0.99±0.66 at 1 week, 1.47±1.17 at 1 month, and 1.49±1.03 at 3 months. The difference between pre- and post-therapy OSI values was statistically significant (P<0.05). The MTF cutoff frequency decreased from (36.6±12.0) cpd before therapy to (34.2±8.9) cpd at 1 week, (27.9±12.6) cpd at 1 month, and (27.7±11.6) cpd at 3 months after therapy. The difference in MTF cutoff frequency between before and 1 month after therapy was statistically significant (P<0.05). The 100% CVA, 20% CVA, and 9% CVA all declined after therapy, but there was no statistically significant difference between before and after therapy. The OSI of tear film quality was 1.27±1.12, 2.09±1.67, 2.07±1.12, and 2.10±1.33, respectively, and the difference was statistically significant between before and after therapy (P<0.05).
After 3 months of orthokeratology, the corneal central astigmatism remained, and the e value became negative, while OSI increased, and MTF cutoff frequency and tear film quality declined. The alteration was more and more obvious along with the therapy within 1 month, and was not significant within the following month. After short-term corneal reshaping, the ocular optical quality declined in some degree, but the loss was acceptable.(Chin J Ophthalmol, 2015, 51: 32-38)
To compare the corneal subbasal nerve density (SND) changes after laser in situ keratomileusis (LASIK) with a microkeratome and a femtosecond laser.
Prospective clinical study. Sixty eyes of thirty myopes were included. Fifteen patients (30 eyes) underwent LASIK with the Moria II microkeratome, and the other 15 patients (30 eyes) with the 60 kHz IntraLase femtosecond laser. Central, temporal and nasal corneal SNDs were measured by confocal microscopy and compared before surgery, 1 month, and 3 months after surgery. Analysis of variance and t test were used for comparing the differences between different time points and two groups.
Preoperatively and 1 month, 3 months postoperatively, the SNDs were (16 728.30±4 300.30), (1 875.42±300.50) and (1 701.55±194.11)μm/mm2 in the central cornea, (11 379.70±1 833.92), (1 341.20±288.68) and (1 860.87±147.60)μm/mm2 in the temporal cornea, and (8 506.79±662.83), (7 428.96±712.99) and (8 044.32±1 077.54)μm/mm2 in the nasal cornea, respectively, in the microkeratome group, and (16 351.59±3 503.88), (1 859.38±452.93) and (2 043.67±377.76) in the central cornea, (12 328.22±2 007.43), (1 483.85±371.28) and (2 126.31±279.87) μm/mm 2 in the temporal cornea, and (8 347.91±789.44), (1 475.53±293.98) and (2 022.10±282.89)μm/mm2 in the nasal cornea, respectively, in the femtosecond laser group. The SNDs at three positions all decreased significantly at each time point postoperatively compared to the baseline values in both groups (1 and 3 months in the microkeratome group: central t=18.981 and 18.912, temporal t=30.121 and 27.921, and nasal t=6.456 and 2.126; in the femtosecond laser group: central t=22.667 and 22.379, temporal t=29.000 and 28.376, and nasal t=46.329 and 41.751; all P<0.01, except 3 months at the nasal in the microkeratome group,P=0.042). The nasal SND increased significantly from month 1 to month 3 (t=-3.921, P<0.01) in the microkeratome group, and the temporal and nasal SNDs both increased significantly from month 1 to month 3 (t=-9.629 and -6.645, both P<0.01) in the femtosecond group. There were no significant differences in the central SND between the two groups (F=0.002, P=0.96). Significant differences were found in the nasal SND between the groups at 1 month and 3 months (t=42.281 and 29.608, both P<0.01), and in the temporal SND at 3 months after surgery (t=-4.595, P<0.01).
Peripheral corneal nerve recovery occurred at 1 month after LASIK surgery. Patients with a femtosecond laser showed better corneal regeneration than those with a microkeratome. (Chin J Ophthalmol, 2015, 51: 39-44)
To evaluate the safety, effectiveness and compliance of Bronuck (bromfenac sodium ophthalmic solution 0.1%) following LASEK in comparison with glucocorticoids.
In this prospective trial, 60 patients (120 eyes) undergoing LASEK were randomized into the bromfenac sodium group (60 eyes) and control group (60 eyes). Patients in both groups initially received dexamethasone 0.1% four times a day after LASEK for 7 days, and then the patients in the bromfenac sodium group were given Bronuck twice a day for the next 11 weeks, while the patients from the control group were given fluorometholone 0.1% with gradually decreased doses during the same period. Results of the routine examinations done before and 3, 10, 30, 90 and 180 days after LASEK were recorded, including uncorrected visual acuity, best corrected visual acuity, intraocular pressure (IOP), corneal topography, ocular symptoms and signs, which were used for comparison between the two groups. All data of right eyes were analyzed for their independence, normality and homogeneity of variance. Independent samples t-test or non-parametric Mann-Whitney test was performed accordingly.
There was no statistically significant difference in IOP and corneal topography (K1, K2, SAI, SRI and CY) between the two groups postoperatively. The IOP was (16.33±6.21) mmHg (1 mmHg=0.133 kPa), (15.67±2.82) mmHg, (15.35±2.22) mmHg and (13.10±3.41) mmHg in the bromfenac sodium group, and (16.87±3.68) mmHg, (14.05±2.23) mmHg, (14.39±2.22) mmHg and (13.18±2.49) mmHg in the control group at 10, 30, 90 and 180 days, respectively. The bromfenac sodium group showed significantly better uncorrected visual acuity (5.16±0.08) than the control group (5.02±0.09) on day 30 (t=2.32, P<0.05). In the bromfenac sodium group, four eyes had visual fatigue, and four eyes had dry eye symptoms on day 180. Epithelial flaps were all well positioned with satisfying healing process. Each group had one case (two eyes) of haze on day 30, and the bromfenac sodium group had another case (2 eyes) of new-onset haze on day 60. But all the cases of haze were graded 0.5 according to the Fantes Standard, too mild to compromise their visual acuities, and were resolved after frequent topical medication for 1 month. Four patients from the control group were prescribed antiglaucoma medications due to elevated IOP. The refractive status remained stable for patients from both groups.
Administration of Bronuck can reduce the amount of or partially substitute for corticosteroids. Mild haze early after LASEK may disappear after intensive treatment. (Chin J Ophthalmol, 2015, 51: 45-50)
To observe the efficacy of bromfenac sodium eye drops on relieving the irritative symptoms after LASEK surgery.
Sixty-four people who had received LASEK surgery were randomly divided into two groups, observing the right eye for each group. group A was given 0.1% bromfenac sodium eye drops twice a day in three days before surgery and one day after surgery; group B was given 0.5% ketorolac tromethamine (acular) eye drops four times a day in three days before surgery and on day after surgery. In the 1st, 3rd, 5th and 7th day after surgery, irritative symptoms grade, duration of irritation, time for corneal epithelial healing, and uncorrected visual acuity were observed and compared between the two groups.
0.1% no discomfort in group A with bromfenac sodium eye drops was observed while 0.5% ketorolac tromethamine eye drops caused tingling, burning discomfort that lasted for 2-3 seconds in 16 of the 28 subjects (87.5%). No significant difference was observed between the irritation grades of group A and B (Z=-1.625, P=0.104); the duration of irritative symptom was significantly shorter in group A than that in group B (Z=-2.895, P=0.004). No significant difference was observed between the time of healing and visual acuity recovery of the two groups.
0.1% bromfenac sodium eye drops can effectively relieve the post-LASEK irritative symptoms, and it is better tolerated than 0.5% ketorolac tromethamine eye drops. (Chin J Ophthalmol, 2015, 51: 51-54)

The patient was a 47-year-old male. He was hospitalized to Tianjin Eye Hospital on July 10, 2012 due to progressive vision loss in his left eye for 1 1/2 years, eye pain for 7 months and vision loss for 4 months. The patient had been seen in an eye hospital on February 22, 2011 due to blurred vision. At that time, the visual acuity of his left eye was 0.1. The edema and fine hard exudation of the macular area were found by OCT, and the edema and detachment of the macular neuroepithelial layer of the left eye were found. Central serous chorioretinopathy of the left eye was considered. Treatment such as aescin and digitalis diglycoside eye drops was given. By December 7, 2011, the left eye was painful, the vision was manual in front of the eye, and the fundus of the eye could not be peeped in. The intraocular pressure was 13.8 mmHg (1 mmHg =0.133 kPa) in the right eye and 32.6 mmHg in the left eye. Vitreous hemorrhage in the left eye and secondary glaucoma were considered, and anti-glaucoma drugs such as brinzolamide eye drops were given. Intraocular pressure continued to rise to 49.3 mmHg. In order to relieve the left eye pain, the left eye ciliary body diode laser photocoagulation was performed twice in December 2011 and January 2012, and the eye pain improved slightly. In March 2012, I went to the original hospital. The visual acuity of my left eye was no longer light sensing, the iris was neovascularized, and the intraocular pressure was 44.9 mmHg. For further treatment, I came to see a doctor. Eye examination: Right eye visual acuity 0.8, left eye amaurosis. Multiple black flat protuberances can be seen from 1 to 5 o'clock in the temporal sclera of the limbus (

About 1/2 PD round subretinal hemorrhage was observed in the macular area of the left fundus, and the surrounding retina showed a gauze mist-like appearance (
Blue light, with the wavelength between 400 nm and 500 nm, has caused public concern because of the injury to the retinal cells. Meanwhile, it is important in circadian rhythm regulation, scotopic vision and ocular growth. Is the blue light safe? Should it be eliminated from the daily life? Here we review the effect and safety of the blue light.(Chin J Ophthalmol, 2015, 51: 65-69)
Reticular pseudodrusen (RPD) has recently been identified as a particular yellowish interlacing network of oval or round lesion in the fundus of aged retinal degeneration patients. RPD can be easily misdiagnosed as typical drusen. However, a large number of observations indicated that RPD and typical drusen are different in distribution, morphological features and pathophysiological processes. The diagnosis of RPD relies on multiple fundus examinations including fundus autofluorescence, infrared reflectance and spectral-domain optical coherence tomography. RPD may be associated with age-related macular degeneration, choroidal neovascularization and geographic atrophy, but the prevalence of RPD is found low in polypoidalchoroidalvasculopathycases. Differential diagnosis of RPD and drusen may affect the treatment and prognosis of these conditions. Thus a careful long-term follow-up is mandatory for these patients.(Chin J Ophthalmol, 2015, 51: 70-73)
