Objective

To construct the fear psychological intervention program for patients with kinesiophobia after heart valve replacement (HVR) and conduct its preliminary clinical application.

Methods

A research team was established to develop the preliminary draft of the fear psychological intervention program for patients with kinesiophobia after HVR through literature retrieval, and the fear psychological intervention program for patients with kinesiophobia after HVR was finalized by expert letter consultation. The recovery rate of expert consultation questionnaires was used to evaluate the expert positivity degree; the expert authority coefficient (Cr) was adopted to assess expert authority; Kendall's coordination coefficient and coefficient of variation were applied to evaluate the coordination degree of expert opinions; the expert agreement rate was used to judge the rationality of the fear psychological intervention program for patients with kinesiophobia after HVR. A total of 24 valvular heart disease (VHD) patients complicated with kinesiophobia after HVR who were treated in Guizhou Provincial People's Hospital from September to November 2025 were enrolled as research subjects. According to the intervention method, the patients were divided into the control group and the intervention group, with 12 cases in each group. The control group received routine nursing intervention, while the intervention group was given the fear psychological intervention program for patients with kinesiophobia after HVR. The intervention course lasted for one month in both groups. The scores of the Chinese version of Tampa Scale for Kinesiophosia Heart (TSK-SV Heart) , Numerical Rating Scale (NRS) , Chinese version of General Self-Efficacy Scale-Schwarzer (GSES) , Self-Rating Anxiety Scale (SAS) and Chinese version of Perceived Social Support Scale (PSSS) before and after intervention were compared between the two groups.

Results

The questionnaire recovery rates of the first and the second rounds of expert letter consultation were both 100.00%, the Cr values were 0.953 and 0.960 respectively; Kendall's coordination coefficients were 0.10 (χ²=101.035, P < 0.001) and 0.12 (χ²=129.008, P < 0.001) respectively, the coefficients of variation of each item ranged from 0.04 to 0.26 and 0.04 to 0.21 respectively, and the expert agreement rates were 42.86% and 4.76% respectively. Based on the results of the first round of expert letter consultation, 1 tertiary item was deleted, 6 tertiary items were added, and 5 tertiary items were revised; 1 tertiary item was modified according to the results of the second round of expert letter consultation. Finally, the fear psychological intervention program for patients with kinesiophobia after HVR was cofmed, it included 4 primary items, 12 secondary items and 37 tertiary items. After intervention, the scores of the Chinese version of TSK-SV Heart, NRS and SAS in the intervention group were significantly lower than those in the control group, while the scores of the Chinese version of GSES and the Chinese version of PSSS were significantly higher than those in the control group (P < 0.05) . In the control group and the intervention group, the scores of the Chinese version of TSK-SV Heart, NRS and SAS after intervention were lower than those before intervention, while the scores of the Chinese version of GSES and the Chinese version of PSSS were higher than those before intervention, respectively (P < 0.05) .

Conclusion

The fear psychological intervention program for patients with kinesiophobia after HVR constructed in this study contains 4 primary items, 12 secondary items and 37 tertiary items, which is of good scientificity and reliability. It can effectively reduce the level of movement fear in patients with kinesiophobia after HVR, relieve pain and anxiety, and improve self-efficacy and social support level.
