China has become the country with the largest aging population in the world. The sixth census shows that there are 178 million elderly people over 60 years old in China, accounting for 13.26% of the total population. At present, the prevalence of Alzheimer's disease (AD) among the elderly in China is 3% ~7%[
Alzheimer's disease (AD) is a neurodegenerative disease with cognitive dysfunction as the core clinical symptom. It has become an important disease that seriously affects the quality of life and physical and mental health of the elderly because of its latent onset, difficulty in early diagnosis and recognition, and limited intervention measures and treatment. According to the Global Alzheimer's Disease Report published by AD International in 2016[
vascular parkinsonism (VP) is a type of secondary parkinsonism. It was first described and named arteriosclerotic parkinsonism by Critchley in 1929, but the role of vascular factors in parkinsonism was controversial in the subsequent decades. With the progress of clinical pathology research, imaging techniques such as CT and MRI can also clearly show white matter lesions, cerebral infarction and cerebral hemorrhage, and the role of vascular factors in Parkinson's syndrome has gradually been recognized. In recent years, VP has attracted increasing attention, but at present, there is a lack of standardized diagnostic criteria and treatment options for VP in the world, and there is little evidence in clinical research and lack of evidence in domestic research. With the increase of the elderly population in China, the number of patients with diseases such as stroke and hypertension is also increasing, and the prevalence of VP will further increase. In order to better standardize the diagnosis and treatment of VP in China, we formulated an expert consensus on the diagnosis and treatment of VP in China based on relevant literature and clinical research results at home and abroad, and introduced the epidemiology, pathology, clinical manifestations and imaging characteristics of VP, so as to better guide clinical practice and standardize the diagnosis and treatment of VP.

Alzheimer's disease (AD) is a common degenerative disease of the central nervous system in the elderly. Its characteristic pathological changes include amyloid plaques formed by abnormal aggregation of amyloid-β (A β) between nerve cells and neurofibrillary tangles (NFTs) formed by abnormally phosphorylated microtubule-associated protein (tau) aggregation in cells. mammalian target of rapamycin (mTOR) is a highly conserved serine/threonine protein kinase, which plays an important role in cell growth, proliferation and metabolism. Abnormalities in the mTOR signaling pathway are closely associated with many diseases, such as cancer, metabolic diseases, and neurological diseases. A large number of in vitro and in vivo studies have confirmed that the abnormal increase of mTOR activity is an important reason for inducing a large number of A β and tau protein aggregation, while mTOR inhibitor rapamycin can alleviate the pathological changes of AD and improve its cognition level. We combined recent findings to explore the mechanism of action of the mTOR pathway in AD and the potential therapeutic effects of mTOR inhibitors on AD.

Nervous system antigen-specific autoimmune disease is a subacute onset that can involve the entire nervous system and is sensitive to immunotherapy. The presentation of subacute or occult onset makes it susceptible to misdiagnosis as infectious, degenerative, demyelinating, neoplastic or vascular disease. Detection of specific IgG in neurons, glial cells, skeletal muscle in serum or cerebrospinal fluid can help in diagnosis and guide the selection of appropriate treatment options. The occurrence of paraneoplastic syndrome is related to the immune response stimulated by neural antigens produced by occult tumors, and the abundant autoantibody spectrum predicts the possibility of high cancer incidence. Therefore, the detection of nervous system-specific autoimmune antibodies can not only help in diagnosis, but also provide clues for clinicians to detect occult tumors early.
