This article is the first worldwide report that lifestyle interventions can reduce the risk of cardiovascular events in people with impaired glucose tolerance (IGT). This study included 577 adults with IGT diagnosed by oral glucose tolerance test from 33 clinics. They were randomly assigned to a control group or one of three lifestyle intervention groups (diet, exercise, diet plus exercise) in a small group. The intensive lifestyle intervention lasted from 1986 to 1992. After the study, the subjects returned to the conventional medical system. In 2016, researchers conducted a 30-year follow-up survey of subjects to assess the impact of lifestyle interventions on the incidence of diabetes, cardiovascular events, microvascular events, cardiovascular death, all-cause death, and life expectancy in people at high risk of diabetes. During the 30-year follow-up, compared with the control group, the intervention group delayed the onset of diabetes by 3.96 years (median), decreased cardiovascular events by 26%, compound microvascular events by 35%, cardiovascular deaths by 33%, and all-cause deaths by 26%. Compared with the control group, the intervention group increased the median survival by 4.82 years and the average life expectancy by 1.44 years. The incidence of stroke and retinopathy in the intervention group decreased by 25% and 40%, respectively, compared with the control group, and the incidence of coronary heart disease and hospitalization due to heart failure, as well as renal disease and neuropathy were also lower than the control group, but there was no statistical difference. Studies have demonstrated that lifestyle interventions in people with IGT can reduce severe diabetic large and small vascular complications and diabetes-related deaths.
