Uterine fibroids are benign tumors formed by the proliferation of uterine smooth muscle tissue, and are the most common benign tumors in women. The incidence of uterine fibroids is difficult to accurately count. It is estimated that the prevalence of women of childbearing age can reach 25%, and the incidence according to autopsy statistics can reach more than 50%. At present, there is no consensus or guideline on the diagnosis and treatment of uterine fibroids in China.
maternal mortality rate (MMR) and perinatal mortality rate (PMR) are the only and very important indicators for assessing obstetric quality in the past. From 1990 to 2015, the global total MMR decreased by 44% (from 385 per 100,000 to 2.16 million per 100,000)[
chronic pelvic pain (CPP) is a group of diseases caused by various functional and organic causes, with pain in the pelvic cavity and its surrounding tissues as the main symptom, and the course of disease exceeds 3 to 6 months[
A 27-year-old patient underwent cesarean section in a foreign hospital in June 2012. During the operation, a right mesangial mass was found, the size of 7 cm x 5 cm x 4 cm; Postoperatively, the pathology department of Qilu Hospital of Shandong University was consulted and diagnosed as low-grade serous carcinoma of the right mesangial tube. One month later, he was admitted to our hospital for right adnexectomy + left fallopian tube resection + omentectomy + pelvic and abdominal para-aortic lymphadenectomy. No tumor was found in postoperative pathological examination, and he was treated with paclitaxel + cisplatin (TP) chemotherapy for 3 courses. Two years later, the patient had abdominal distension, and B-ultrasound examination showed cyst in the right adnexal area and cystic mass in the posterior uterus; Pelvic CT showed multiple cystic lesions in the pelvic cavity; Serum CA125The level was 15.47 kU/L. The outpatient clinic receives "pelvic mass" in our hospital. Gynecological examination: a mass with a size of 5 cm ×4 cm can be palpable behind the uterine body. Laparoscopic exploration revealed extensive adhesion of the left ovary to the posterior lobe of the broad ligament and sigmoid colon, and two cystic-solid masses in the recto-uterine depression. Intraoperative rapid frozen pathological examination revealed granulosa cell tumor, which was changed to total transabdominal hysterectomy + left ovariectomy + retroperitoneal lesion resection. Postoperative pathology: (recto-uterine depression, mesenteric) juvenile granulosa cell tumor, no tumor in uterus and left ovary. Immunohistochemical detection: CD99, calcium binding protein (CR), vimentin (vimentin), CD56, cytokeratin (CK), Wilms tumor gene 1 (WT-1) were all strongly positive (+ + ~ + + +), α-inhinbin (α-inhinbin), p53, CK7 were scattered positive (+), cell proliferation-related nuclear antigen (Ki-67) 10% ~15% (+), synaptophysin (Syn) was weakly positive (±), CA125CD117, CK20, and chromaffin A (CgA) were all negative (-). Two consultations with the Pathology Department of the Cancer Hospital of the Chinese Academy of Medical Sciences showed that: (right mesangial tube, rectal uterine depression, mesenteric) malignant tumor, the tumor cells are short fusiform, arranged in a solid, tubular, microcapsular sieve shape (
Endometriosis (endometriosis) is a common disease in women with hormone-dependent and organ-dependent characteristics. According to the 2015 Guidelines for the Diagnosis and Treatment of Endometriosis[
Epithelial ovarian cancer (ovarian cancer) is the most lethal malignant tumor of the female reproductive system. Cancer statistics in the United States show that in 2015, there were 21,290 new cases of ovarian malignant tumors and 14,180 deaths in the United States[
Endometriosis (endometriosis) is one of the common obstetrics and gynecology problems, a group of syndromes, a chronic disease. Scholars have conducted a lot of research and accumulated rich experience. In 2007 and 2015, respectively, the "Standards for the Diagnosis and Treatment of Endometriosis" (Chinese Journal of Obstetrics and Gynecology, Issue 9, 2007) and "Guidelines for the Diagnosis and Treatment of Endometriosis" (Chinese Journal of Obstetrics and Gynecology, Issue 3, 2015) were released, which promoted the standardized diagnosis and treatment of endometriosis. The number of experts participating in the 2015 "Guide" increased from the original 15 to 30, taking into account all aspects of technological development, especially in the management of pain, mass and infertility, and also in-depth elaboration on surgery, deep infiltrative endometriosis (DIE), malignant transformation and postoperative management. In view of this, we have organized some experts to carry out focused and delicate interpretation for the reference of fellow peers.
The 13th World Congress on Endometriosis (WCE) was held in Vancouver, Canada on May 17-20, 2017. The conference presented, discussed and exchanged major scientific issues such as the epidemiological characteristics, pathogenesis, clinical diagnosis, treatment and endometriosis infertility of endometriosis (endometriosis). The content features of this conference include: further research on the pathogenesis of endometriosis; A deeper understanding of the nature of endometriosis; Pay attention to the immune abnormalities and comorbidities of patients with endometriosis; Pay attention to the quality of life of patients, especially abnormal sexual life; Focus on basic and assisted reproductive treatment of endometriosis infertility; Drug and surgical treatment of endometriosis remained one of the main contents of the conference.
