About 50% of pregnant women in the first trimester of pregnancy will have nausea and vomiting, 25% will have nausea only without vomiting, and 25% will be asymptomatic. gonadotropin releasing hormone (GnRH) is a neurohormone secreted by the hypothalamus. It is a peptide hormone composed of 10 amino acids. Its main physiological function is to release LH and FSH, which plays an important role in the regulation of vertebrate reproduction. GnRH analogs include gonadotropin releasing hormone agonist (GnRH-a) and gonadotropin releasing hormone antagonist (GnRH-ant). GnRH-a is a decapeptide analog of GnRH, which can bind to pituitary GnRH receptor and promote the release of FSH and LH. Continuous application can inhibit the synthesis and release of pituitary gonadotropin (Gn) and ovarian gonadal hormone. However, GnRH-ant is mainly modified at the 6th and 8th amino acid sites of GnRH decapeptide, which prolongs the half-life, binds to the GnRH receptor of the pituitary gland but does not exert biological activity, and can completely block the effect of endogenous GnRH, causing the levels of FSH and LH in serum to decrease rapidly, and the LH peak cannot appear. Compared with GnRH-a, GnRH-ant has the advantages that it has no "flare-up" effect, can quickly inhibit the release of endogenous LH, and does not need pituitary desensitization, so the dosage of Gn is less than that of GnRH-a cycle; In addition, GnRH-a can be used instead of hCG to induce ovulation in the ovulation induction cycle of GnRH-ant, which can reduce the occurrence of ovarian hyperstimulation syndrome (OHSS). In recent years, the demand for clinical application of GnRH-ant in assisted reproductive therapy has gradually increased, but most clinicians have insufficient initial experience and long learning curve, so they need technical guidance and standardization urgently. Therefore, this consensus was drawn up by the Reproductive Medicine Branch of Chinese Medical Association organized by experienced experts in China to discuss and recommend it to clinicians for reference.

With the increase of cesarean section rate, many clinical problems have attracted more and more attention, such as uterine rupture, placenta previa, placental implantation, cesarean scar pregnancy (CSP) after cesarean section, infertility, intraabdominal organ adhesion, etc. Among them, CSP, as one of the important early pregnancy complications of women after cesarean section, has become a common new type of obstetrics and gynecology emergency. CSP refers to the implantation of the gestational sac in the uterine scar or the diverticulum formed in the previous cesarean section, so it can be considered as a special type of ectopic pregnancy. As a long-term complication of cesarean section, clear and unified guidance is urgently needed in its diagnosis and treatment. CSP has become a "new disease" and has received unprecedented attention, and the number of related academic articles published at home and abroad continues to rise. The types of articles also range from early case reports to more recent series of studies with increasing case numbers. Under the historical background of high cesarean section rate in China, the incidence of CSP may be particularly serious. Therefore, it is more urgent for scholars to continue to study and summarize in depth, and formulate industry norms in line with China's characteristics, so as to improve the diagnosis and treatment level of CSP and the quality of perinatal health care. This article focuses on one of the most serious and difficult problems in CSP, that is, the dangerous placenta previa with placenta implantation caused by CSP continuing to develop in the first trimester of pregnancy. Moreover, the focus of this article is not to explain the placenta implantation in the second half of pregnancy, but the related problems related to CSP, placenta implantation in the first trimester and the first trimester of the second trimester of pregnancy, so as to strengthen clinicians' understanding of this serious obstetric complication, so as to identify and properly deal with it in clinic as soon as possible.
