congenital adrenal hyperplasia (CAH) is a group of autosomal recessive hereditary diseases caused by defects of various catalytic enzymes at various stages of the adrenal cortical steroid synthesis pathway, mainly causing corticosteroid synthesis disorders. The most common CAH is 21-hydroxylase deficiency (21-OHD), which has the risk of fatal adrenal salt loss crisis, and hyperandrogenemia causes growth and gonadal axis disorders. Female, 4 d, was admitted to the Department of Neonatology, Liuzhou Maternal and Child Health Hospital, Guangxi Zhuang Autonomous Region in 2013 because of "1 d poor feeding". Auxiliary examinations: blood routine, C-reactive protein (CRP), cerebrospinal fluid examination was normal, blood culture was negative, chest X-ray: both lung texture increased, heart shadow increased (cardiothoracic ratio 0.72), heart color ultrasound: patent foramen ovale, mild to moderate tricuspid regurgitation, pulmonary hypertension. Treatment and diagnosis: The child was treated with piperacillin and tazobactam for anti-infection. On the second day of admission, the child developed shortness of breath, fine wet rales could be heard in the lungs, and the heart rate fluctuated between 160 and 180 beats/min. Considering heart failure, the child was treated with cardiotonic, diuretic, and circulation improvement.
