Dynamic cervical X-ray is often used to evaluate fusion after anterior cervical discectomy, decompression and fusion (ACDF) because of its accuracy similar to CT in evaluating postoperative fusion. To explore the ability of cervical dynamic position radiographs to predict revision surgery due to asymptomatic pseudoarthrosis, the effect of fusion on adjacent segment movement, and whether there are differences in clinical efficacy between asymptomatic and symptomatic pseudoarthrosis patients in patients after ACDF, Lambrechts et al. retrospectively analyzed 597 patients (1,203 segments in total) who underwent 1-to 4-segment ACDF surgery in the author's hospital in the past 10 years. The movement of each segment after ACDF was evaluated by using the range of spinous process movement on flexion and extension X-ray. Pseudoarthrosis was defined as spinous process movement>1 mm in flexion and extension X-rays. Based on the magnitude of interspinous motion at each fixed segment, a subject operating characteristic (ROC) curve was plotted to predict the probability of occurrence of pseudoarthrosis. Patients' preoperative and postoperative neck and arm pain visual analog scale (VAS), neck disability index (NDI), and modified Japan Orthopedic Association Scale (mJOA) were recorded, and patients' clinical recovery was assessed with SF-12, MCS-12, and PCS-12 scores.
To explore the efficacy and safety of flared decompression (TSD) in anterior cervical discectomy, decompression and fusion (ACDF), Dong-Ho Lee's team from the Department of Orthopedic Surgery, Asan Medical Center, Ulsan University School of Medicine, Seoul, South Korea, evaluated imaging and clinical data from at least 1 year follow-up of 227 patients undergoing ACDF due to cervical spondylosis or cervical radiculopathy due to ossification of the posterior longitudinal ligament. Patient characteristics, fusion rate, subsidence rate, visual analogue scale of neck pain (VAS), arm VAS, and cervical dysfunction index (NDI) were compared between patients receiving TSD (TSD group) and patients not receiving TSD (non-TSD group).
