Intracranial atherosclerotic stenosis is the cause of ischemic stroke with high incidence in Asian population, and the proportion can be as high as 46.6%. In western countries, the proportion is only 10% to 15%. Endovascular therapy such as interventional stenting has been regarded as a potential treatment for intracranial atherosclerotic stenosis. The SAMMPRIS and VISSIT studies are the only two multicenter randomized controlled studies comparing stents with drug alone in the world, both mainly in the Western population. The results show that stent therapy has a higher complication rate, which is worse than drug therapy alone. In recent years, many prospective registry studies and real-world studies at home and abroad have shown that the risk of intracranial arterial stent complications is only 2.0% ~4.3%, which is in sharp contrast with the 14.7% in the SAMMPRIS study. Therefore, therapeutic strategies for intracranial atherosclerotic stenosis are controversial.
